Start off at the EFMP office on base. They will provide you with all the forms that need to be filled out for the family member with the special need.
There is a portion that you or the service member will need to fill out and sign. Then you take it to the medical care provider and they will fill out their portion. I have always taken our EFMP forms directly to the specialists that treat our EFMs since they know exactly how many appointments, etc are needed. I have a friend who always takes her to the primary care physician and he fills it out. Ask at your EFMP office which way is best for them.
After the paperwork is filled out, turn it in to the EFMP office and they will process the paperwork. If accepted, you will be assigned a category and a case worker that will be able to answer questions and help you get access to the things you need.
In the Marine Corps EFMP, the paperwork needs to be renewed every 3 years. I am unsure of other branches.
