I got one of the covers for the changing pad that is like a fleece material, but I feel like there should be something waterproof on top of that so I won't have to wash the cover really really often.
Kinda the same question for the pack n play - there's the mattress that comes with the pack n play, and there are sheets sold separately, but shouldn't there be something in between the sheet and mattress?

for the changing pad, I have a waterproof pad that I laid on top of the cover. I'd lay DD on the pad to change her. That way if there's an accident, I only change the pad.
For PNP, I didn't use anything inbetween the sheet and mattress. But when she was little, if she slept in it (or if I used the changing table attached to it), I would also lay one of those waterproof pads down. They're pretty handy when they're little.
This is what I'm referring to.
I used a lap pad on the changing table in the beginning. I don't use it anymore, but I keep the cover off half the time...I've gotten a bit slack.
I think I used something similar in the PNP (or maybe just a cloth diaper - the burp cloth kind), but usually just under his head b/c he spit-up so much. Oh I do not miss reflux!
Our changing pad is waterproof so if they got anything on the fleece cover, I would just take it off (I had two for this reason) and wipe off the changing pad before putting the clean fleece cover over it. I don't see the point in having a plastic changing pad, fleece cover plus another liner over those.
For the PNP, we also only used sheets. The PNP "mattress" is plastic material that wipes clean if anything gets on it. We did put a plush sheet in between the mattress and a regular sheet but that was only to add more padding for comfort.
When Savannah was really little I would use these Huggies disposable pads on top of the fleece cover. I hadn't planned on using anything like that but my mom bought me a buttload of them (I actually have plenty left for baby #2). It was nice because at least once a day she would pee or poop when we were changing her.
I used Phil & Ted's Traveller for my PNP whenever we went out of town (hated it, by the way). I just folded and wrapped a regular sheet around the mattress (nothing between). In her bassinet and crib I would sometimes use a burp cloth under her head because she would spit up so much.
