Have you looked on the formula company's website? It they don't have the information I would just slowly start replacing her formula bottles/cups with cows milk. Also, if you don't hear back from anyone perhaps post your question here.

I just combined milk and formula for a day and then did it cold turkey - DD was fine, didn't miss a beat, I also let the milk sit out for a little while (20ish minutes? to take the chill off too the first few days). I did it when I still had a few days worth of formula around so that incase there was a massive protest or a late night issue, I wouldn't have to buy more formula or be stuck w/ a miserable baby @ 3 am and no formula.
After that, I just dropped bottles slowly and started giving her milk in sippies w/ or after meals. A little after 14 months, she started playing w/ her bedtime bottle, so one night, I didn't offer it and she was fine.
Just go slow and read your LO's cues; no rush to make the transitions as long as you are working on them, I think you are fine.
We just went cold turkey to milk. If Aiden had not seemed to like it or it upset his belly, I probably would have combined both, but he gobbled it right down!
