I definitely think it is the area. While I haven't been as "out and about" since we've moved here (I have DS, so he slows me down a little), I have definitely noticed that at least in NE Indiana that NIP and babywearing is a rare thing to see.
A little background, I'm from KY, have lived in rural and urban areas in OH, MD, WV, and DC, and have spent significant time abroad.
I see it but infrequently. As far as NIP, I see it being done discretely. I have seen several instances in BRU. I only NIP a few times in the 11 mos I nursed L.
I was hoping that more people did it than I thought. I've NIP a few times but rarely am I super comfortable with it. I think I'd be more comfortable if I knew people were doing it around me.
My mom saw two ladies NIP at Keystone mall within <1 hour. I thought that seemed like a very rare event. One of them had twins and two older kids. I can't imagine!
I don't know that you would call it NIP, but I have nursed more than once in the "mom room" in the restrooms at Nordstrom or Saks or Macy's. I was never comfortable enough to whip it out on a random bench in the mall.
I don't live near you, but up here in Michigan it totally depends on the neighborhood I'm in. There are two major malls in town, and one is much closer to the hippy area of town. I almost always see babywearing there, but almost never at the other more suburban mall.
I NIP while babywearing all the time! My kiddo is a snacker, so if I didn't NIP I could never leave the house. I am pretty discreet, but probably people can tell that we are nursing.

I've only seen 2 people and I almost didn't notice right away b/c it wasn't obvious. But I don't think a *ton* of people do it in this area.
I have seen a fair amount of babywearing at the grocery and art fairs and places like that.
