Recurrent pPROM fears- anyone talk with OB/MW?

H and I are fairly certain that we're one and done due to my pPROM, PPD, etc. But if I were to get pregnant in the future I'm fairly certain we would be OK with that---except that I'm scared to death of pPROMing again, and earlier next time. I pPROMed at 34w4d last time and never went into labor (they induced me a week later). No infection was ever found, and no reason in general besides the irritable uterus I'd had since week 19. A fFN test done 10 days before my water broke was negative and I had a long, closed cervix. 

Has anyone already had the discussion with your OB or MW on the chance of it happening again? Do the p17 shots help with pPROM if there was no PTL? Would they quell my irritable uterus, in case that was what did us in? 

Thanks for any insight you can provide! I feel odd going to my MW/OB with a hypothetical.

  • I pPROM'd with my daughter at 24 weeks for no known reason.  I didn't have an infection, never had any contractions, wasn't dilated.  I stayed pregnant until I was induced at 34 weeks.  For my second pregnancy I was on the progesterone shots and had several ultrasounds to check cervical length.  My OB also put me on modified bedrest starting at 21 weeks.  The combo of all this worked extremely well, I just had my son 2 weeks ago at 39 weeks.

    Every pregnancy is different, I'm proof of that.

  • I talked to my MFM about it while in the hospital....I just had my baby due to pProm a month ago after 7 weeks in the hospital.  My water broke at 25W6D and I had her at 33W1D, so it's something I would also prefer not to do again either!

    But, for me, there was never any reason why my water started leaking so early, I had no other issues or complications.

    He basically said that there was no indication that it would happen again in my next pregnancy but there was always the risk especially since it happened the first time.

    Hubby and I have basically decided that when we decide to try again we have to be mentally prepared to go through all of this again just in case and if we aren't mentally prepared we will not try yet.  It may be 4 or 5 years before we will be mentally ready to go through this whole 3 month ordeal again though....

  • I ruptured at 23w1d and had little to no measurable fluid until I delivered at 27w3d.  I also had no signs of infection, no contractions, nothing.  My pregnancy had been textbook up until that point.

    My OB has said more than once that there is no reason to think I couldn't carry a subsequent pregnancy to term.  Great.  But there was no reason to think I couldn't do that in the first place...and we all know how that played out.  If I were to get pg again, my OB would give the p17 shots and I would of course be considered high risk.

    With all that said, I think we're one and done.  Though we never planned on more than one in the first place.

  • I had that discussion with the MW at my 6 week PP appt lol. I never got a reason for my PPROM, either, and my MW told me that the odds of it happening again are about the same as when it happened the first time. That being said, it did happen the first time, so I'm definitely nervous about it. I plan to ask about the p17 shots when I do get pg again. I'm trying really hard to just give this fear to God, but it's been a struggle lately. I'm going to focus on getting pg first, then I'll worry about PPROM.
  • I PROM'ed at 32 weeks with my son.  Had an irritable uterus the entire pregnancy.  

    With my second pregnancy, the irritable uterus has started earlier, which my doctor says is normal, but can also lead to PROM in people who are susceptible to it.   I am on the P17 shots.

    My husband and I went into this 2nd pregnancy with the mentality that we could handle another preemie, but we don't obviously want our second child to have to go through that.  

     

  • My MFM said that most cases 're-occur in the same manner they first happened' and that p17 shots would help a lot. We are moving forward with having a 2nd.
  • I pPROM'd with my first at 33 weeks 0 days and like you and the previous posters, no "reason" was found.

    I am currently 35 weeks with my second and I've been on p17 shots since 13 weeks.  I do contract A LOT but they aren't productive, meaning they aren't changing my cervix.  In my practice, the dr.'s seem to think that "repeaters repeat," so I am watched closely.
