Supplementing with Formula

Is it okay to mix pumped bm and formula in the same bottle??  And if I do how much of each should I do (ie if DD usually takes 6ozs BM can I give her 4 ozs BM and 2 Formula??)

Yes you can mix them together, however you have to follow the more limiting time rules (you should finish the bottle within 1 hour of first feeding, and use it within 24 hours).  You can mix any combination.  I've had to do that since I've used up the last of my frozen supply (I'm weaning from daytime feedings).
