It might depend on the doctor, but I see Dr. Somer-Shely and had my first appointment at 9 weeks and was told that she does not normally do an U/S at the firsst appointment. I called a couple days before my appointment and told them I was really anxious and asked if they could please do one just to reassure me and they did a quick one for me-not an "official" one with measurements, etc., but she just brought in a portable machine and showed me there was a baby in there and that everything looked normal.
I don't know for sure if they do for everyone. I know I will have had an ultrasound by then, to determine due date. I am also planning on delivering at the new hospital, it's just a few miles north of where I live.
