We had our MFM appt on Friday!  We have a plan!!
1) Do a 24 hr urine sample and blood test (I had protein in my urine)
2) Do an u/s to look for any abnormalities before scheduling the
cerclage. I've never had a non-preg u/s.  He said my cervix (or
uterus?) is to the left side, which could be nothing or something :-)
They want to make sure something isn't responsible for my Incompetent
cervix that would repeat itself. We'll do this in 2 weeks!
3) I'm probably a candidate for either cerclage.  Given that I was
showing signs of dilation in the 23rd week, and usually (not always)
the IC strikes sooner in the 2nd preg (BOOO), the risk/reward of
the abdominal cerclage is far more attractive than than a vaginal
cerclage.  (At least in our opinion and the doctor's.)
4) We have to talk about it, but I think we're going for the
trandabdominal cerclage.  The doctor has done about 10% of TACs in the
entire COUNTRY.  Love.  He seems to be very forward thinking.  But, he
*just* started doing them before pregnancy a few months ago.  He said
before pregnancy things are tight and hard to manipulate and tie onto -
but after pregnancy has risks of hemmoraging and other things due to
high blood flow in the area.  I would be the 7th patient to have one
day pre-preg, laproscopically, with a robot.  It scares me a little that I will be patient #7 for this specific procedure, but he seems awesome and I trust
him. My friend had a TAC by him (and knows several others) after being pregnant (triplets) with a traditional c-section (not laproscopic)..
5) He's had some failures, complications, and losses, but keeping in mind he sees the highest risk patients (one had 6 babies, several with multiple complications, and many triplets etc), and he said being at a healthy weight decreases my risk.
He rattled off a ton of other details, risks, info, etc. but that's all I have time to write for now.
All in all, things look optimistic, but a lot ahead of us. The u/s will really be telling.

Hey Lemen! Looks like we have a few things in common. With Claire, I began dilating at 23 weeks, but like you, I wasn't always dilated. They called it a "dynamic cervix". Are they using the same term with you? I remember during an ultrasound I actually watched my cervix go from completely closed to slightly dilated to closed again. Weird. And scary! I also had a transabdominal cerclage with pregnancy #2 and although Keira also ended up being a premature birth, my cervix never opened. My premature births also (unfortnuately) have a very strong PTL tendency even after the cervical issues were solved. If IC is your only problem.....I think I see a full term baby in your future! I'm not on this board as much as I used to be, but I do check in so please let me know if you have any questions about the TAC.
