Claiming TDI

I just had my baby on Tuesday and I need to file for TDI.  I'm totally confused how this works.  I had a c-section.  I see the form to fill out online but I'm wondering...

 How much will I receive?

How many weeks will I receive?

I'm a sales rep. and my paychecks change each week depending on my commissions.  How can I calculate the amount I will receive?  The online form asks for enter the gross amount that I receive each week but it changes for me so I don't know what to enter.  Any help?

    They will determine how much you get based on your previous earnings over the past 2 years...they send paperwork to your job who will give them that info.  Don't they ask for the previous week's pay or something like that?  I filled mine out a few weeks ago and can't remember the exact wording but if you are off a little I wouldn't worry about it since they will get the numbers they need from your employer
