High-Risk Pregnancy

Ladies with IC and Cerclage

...did your doctor also order P17 shots and bedrest? I had a second trimester loss in October. I'm pregnant again and I'm with the same doctor. This pregnancy she wants to cerclage me at 13 weeks and that's it. I've read numerous posts where ladies are cerclaged and the  give  P17 shots. I'm considering visiting another doctor. What is your doctor prescribing?

Re: Ladies with IC and Cerclage

  • I had a cerclage at 13 weeks too and am currently 29 weeks.  I have not had to be on bedrest but get cervical exams every two weeks.  I don't exercise as much and I am not on my feet for long.  I haven't had the P17 shots and I think there only for women who are on full bedrest.   I'm interested in what other people have to say.

  • @Preginchgo, my doctor doesn't really see the need for me to be on bedrest but I'm concerned because I have a long commute to work. It sounds like you're doing good.
  • I've been on bedrest since 20w and still going.  Had an emergency cerclage at 23w.  I am not getting the shots-, but am doing 200mg of prometrium (progesterone) vaginal suppositories every night.  Supposedly, there's research that shows since it's absorbed right into the bloodstream in that area, it can help stave off PTL.

My OB said that next time, we will do the cerclage at 13w.  And that the preventative cerclage is stronger- and I can do any physical activity with it.  It reduces the need for bedrest by 80%, and we will do the prometrium suppositories again.  And also be monitored by u/s for cervical length until 28w.

I would just be frank with your doctor and tell them that you want to cover every base given your history.  Ask about the suppositories if they don't want to do the shots...if you don't get anywhere...I would definitely look for someone else- but my OB told me that there are alot of docs out there who won't even do the preventative cerclage until multiple 2nd tri losses.  GL!  Keep us posted.

  • Hmmm, I'm already on 100 mg of endometrium suppositories. I guess I could just keep using them. I'll definately speak with her about the shots.
  • i had a cerclage at 17w3d and have been on modified bedrest since (well actually since 16w4d). i asked my dr. about the shots and he said since i was having no symptoms other than a short cervix (which has held steady since my cerclage) and no contractions it's not something he does.

he did said it is something he does with people who had previous experiences with ptl, which is what i have read in various medical journals.

  • I had a miscarriage at 20 1/2 weeks before I had DD (9 years ago).  I was diagnosed with an IC and had a cerclage with DD at about 13-14 weeks.  I don't remember staying overnight.  I went in the morning and was released in the evening. I was not put on bedrest, but modified bedrest, limited activity.  This time my doctor is suggesting the cerclage around 15 weeks (not sure why this dr likes doing it a little later).  I go to the high risk dr in 2 weeks and will know more.  My OB dr told me the high risk dr is very conservative and overly-cautious and will keep me overnight after the cerclage.  I am not sure what else he will order. GL to you.
  • I was also given p17 shots with my cerclage. I would get a second opinion as well as research it.
  • I also had a late 2nd tri loss last year and I am on prometrium suppositories but did not get a cerclage. However, now I am pregnant again and have a shortening cervix so I am not sure if skipping the cerclage was a good idea. It didn't start to shorten until 26 weeks though. If we are blessed with another baby I will for sure be asking for a cerclage though just to be safe.
  • This is my first pregnancy, so it is different in terms of circumstances than you. But I have only had a cerclage, no shots or bedrest.
  • My doctor mentioned progesterone suppositories, I was going to ask about those vs the shots at my next appt.  No bedrest mentioned yet until any problems occur.  They are checking my cervix every other week already, though.

  • My first pregnancy ended with a loss at just over 15 weeks. This time, our dr. put in a cerclage at 13 weeks.  We are doing well so far! After the cerclage I went to the dr. every two weeks, until they said we were doing so well we could come back in 4 weeks. So, now I am back to going every 4 weeks. My next appt is on Monday. He did talk to me about bedrest if I didn't take it easy. I teach and am on my feet all day, but I have really had to cut back on that. I come home and pretty much put my feet up for the night. Just take it easy and try not to worry too much!!  Cool

  • Thanks for the comments. I think that maybe I'll ask for a modified schedule at work rather than doing bedrest. I work in D.C. and commute from baltimore. Wouldn't be a problem but I take the train and then walk 8 blocks. It can get kind of hectic. I'm thinking maybe I'll just start driving.

My doctor isn't sure if I loss my twins due to IC or what, so the cerclage is a precaution.
