High-Risk Pregnancy

19 weeks, bleeding/clots 4x this week, twins

I don't know what's going on.  I am 19.5 weeks along with b/g twins (from IVF).  Pregnancy has been pretty uneventful so far until last weekend.  On

Sunday - I had bleeding with a quarter sized clot.  No cramping/pain.  Went to ER.  Hearbeats were fine (150s) and ultrasound didn't show any reason for bleeding. 

Tuesday - more bleeding.  Nickel sized clot.  Called triage nurse and she said if no pain, just take it easy & see OB next day (which I had scheduled anyway from Sunday's bleeding).

Wednesday - saw OB and she said things looked fine.  Watching low-lying placenta on Baby A, but all seems fine.  Take it easy and rest.  No lifting, pelvic rest, etc.

Saturday - bleeding again.  Lots of blood this time.  Soaked through undies, jeans and ran down leg to drip on carpet.  Back to ER (they won't see me in OB Triage until 20 weeks...I am 19w 5d).  Jello-like smooth clot size of 4 stamps, if they formed a square formation.  ER doc goes, "Good news!  Heartbeats fine (140s) but maybe body is trying to abort, so fingers crossed."  It really threw me off.  I don't know what's going on but I'm pretty scared.

Sunday - bleeding again.  Same size clot as Saturday but blood pretty minimal.  Did bed-rest all day.  Seeing Perinatologist tomorrow.

I don't know what to think.  Low-lying placenta, placental abruption, venous lake (placental late), miscarriage emminent...I don't know.  I am dying to Perinatologist's ultrasound and for some answers.  (like, is there any evidence of placenta ripping from wall?  Or like, where are the clots coming from?  If it's from the placenta, will it grow back?  Is baby getting enough nourishment/oxygen if he's losing pieces of the placenta?  If baby A does m/c, what happens to baby B?  2 sacks...fraternal)

Don't know what to believe or do until I get some answers.  Has anyone experienced something like this, around this stage in their pregnancy?  What was the result/outcome?  I know we all have different situations, but if anyone wants to share their stories/experience/opinions, maybe it will help me stop speculating.  Just need something to hold onto right now...

  • Hello! I am SO sorry for all of these scares! I know how awful they can be. When I was 30 weeks, I had a massive gush of blood and was rushed to the ER. I knew Baby A already had a complete placenta previa. I guess it aburpted? I still don't know! But I also was having contractions so I received the magnesium, etc, and was admitted for a few days. Two days later, I stood up to walk to the bathroom and had another massive gush of blood. So the OB admitted me on hospital bedrest. She said if I bled one more time that she would just deliver the twins. A couple weeks later, I had another bleed so they did a c-section. But I was already 30 weeks when all this happened, so I can't imagine how scary this must be for you so early.

    Take it easy!!! If you are not on bedrest, you need to put yourself on bedrest. Drink TONS of water! Also, my iron was seriously low from all the bleeding and I needed 4 blood transufsions and IV iron infusions so if you haven't had your iron checked, please do so.

    Good luck and I really hope it is just the placenta giving you trouble! And stay OFF your feet! Lay flat as much as possible.

    We have some results - sort of.  Went to Perinatologist today.  The tech did a lot of looking with internal and external ultrasound.  Then the doctor came in and basically did the same.  They ruled out low-lying placenta/placenta previa, placenta abruption/pulling away of placenta from wall, or any evidence about the baby being anything less than ok (both heartbeats strong, both sacs full of plenty of fluid, etc.)  The cervix is closed tight and nice and long -- the best they'd hope for. 

    So, they can't really tell me what the bleeding is from but they said that under these circumstances (having bleeding), I'm in the best possible scenario they would hope for -- which is not having any reason/complication that they can 'blame' it on.

    The Peri took a lot of time with me and answered my list of 18+ questions I had.  He was assuring but still heeded warning.  He put me on 2 weeks of blood rest and explained that it was good the clots and blood came out because they would not want it staying inside of me, potentially causing an infection.  But, it was also a wake-up call that I was bleeding so he wanted strict bedrest.  Mostly, he wanted me to remain low-stress, and off my feet -- no housework, meal prep, shopping (not even in the electric cart most stores have).  So, I will comply fully.  I don't want to take a chance and -- if something did happen -- he wants to minimize all blame I could put on myself.  Since I am still only at 20 weeks (today), my health is more important so I can make it 24 weeks, minimum, which is the viability age (when the babies could possibly live outside the womb). 

    So, I will be on bed-rest the next 2 weeks and see how we do.  It's frustrating, because I want to get ready/do stuff, etc. but this is so important to me...as I am sure it would be to each of you.

  • That is great news! Although no explanation leaves you scratching your head, at least it isn't bad news!  Good luck on bedrest, I would stay off your feet for the rest of my pregnancy if I were you, but I don't know how feasible that is. Drink plenty of water! And hang in there :)

  • Good grief!!!  I had more bleeding this morning.  I mean, a flat-lemon sized clot (the biggest one yet) just came out with a pool of blood following it out.  How can they not see where this is coming from?  It seems like a lot of stuff!  I am going back to the doctor in an hour....stick babies, stick...
