So my much larger Baby B has went from vertex to transverse. My doc knows I want to attempt a vaginal delivery, but I am worried now that the larger Baby B is transverse. Baby A has always been first in line, head down and the smaller of the two. I hope that Baby B decides to follow the rules and get back to vertex position. 

Would any of you decided to go ahead with a c/s if this happened to you? I'm afraid of the "double whammy" but not petrified. My concern is for the health of Baby B and the complications that can arise, doing a breech extraction, etc, just because she is measuring to be the bigger baby. Has anyone's experiences been similar to this?

  • My OB said that Baby A had to be bigger to have a vaginal delivery (besides being head down.)

    I would probably opt for the section, but I was OK with the idea of a section the whole time anyway. (And wound up with one since they were both breech.)

  • My baby b went from transverse to breech the whole pregnancy. Baby A was always vertex. I had a vaginal delivery and B flipped vertex once A was born. I've heard of B's in vertex flipping breech during labor, so anything can happen.

    My B was always smaller though.

    What does your OB feel comfortable with?

  • My delivery was an emergency c/s so I didn't really have a choice. Baby A was breech and baby B was transverse. If I was in your situation I think I would also be afraid of the "double whammy" and probably opt for the c/s.  My experience was good and the recovery was not half as bad as I expected. Good luck!
  • My OB is comfortable with doing breech extractions. He did forewarn me that because B is bigger, it could be more difficult to get her out.

    If I heard that I would have the c/s, but I never wanted to have my kids vaginally so maybe I'm biased.  The recovery time from my section was minimal...I was up and walking the next day & able to take care of the babies with no problems.

  • Here's my personal thoughts on "bigger B babies". Women deliver big singletons vaginally all the time. And w/o the helpful "prep" of a baby that came out first. So why is it such a big deal w/ twins if Baby B is bigger?

    I guess I never had to deal w/ that issue myself, so I don't have all the facts. But if your OB is fine w/ it and you are prepared for anything, than go for it.

     

  • I am so hoping with the growth ultrasounds I'm getting next week that their sizes level out a little bit. That is part of the reason we're having the consult with the MFM next Friday. I "think" that if they're measuring close in size my doc will still let me attempt a vaginal delivery, but if they are almost a pound apart as they are now, I'm thinking he'll favor the c/s.
  • My doc said he wouldn't do a vaginal delivery is baby B is bigger than baby A.  I never asked why because I had so much stuff running through my head at that moment, but he was pretty concrete on the matter.

  • My babies measured the same at my last growth scan before my induction. Baby B kept flipping back and forth between breech and vertex during those last 7 weeks. Both boys were head-down when my induction started but after I pushed A out the OB did a u/s and B was breech again! (Not sure at what point he flipped; I think right after A came out but it's hard to say.) So the dr. did a breech extraction. My babies were only 3 oz apart (though both almost a pound heavier than last u/s estimate) so it really didn't matter which was bigger.

    The reason it makes a difference w/twins if B is bigger is really just if B flips to breech or transverse after A comes out. That can't happen with singletons and if it's known that the singleton is breech the mom would usually have a C-section. But with twins, you can't know what position Baby B will be in for delivery; it's harder to do a successful breech extraction on a baby who's larger than the one who just came out. I would think it would have to be a pretty major size difference though to really matter.

    That said, as long as your OB is comfortable with vaginal delivery (and hopefully with breech extraction for Baby B if necessary), I would go for it.
