For us, the on-the-go bottle warmer. A great concept, but a bowl of hot water is still quicker. At least it was only around $20, so not a huge waste.
We also bought a crib wedge that turned out to be a complete failure - we tried it out one night to find Dylan had slid slideways down the wedge by morning. We returned it the next day.
The only thing we have that we've never used is our swing. We have the fisher price 3 in one and DS has never been a swing baby. The good things are that A) we didnt' buy it, it was a present and the seat actually comes off the swing part and becomes a baby/toddler rocking chair. So we just have the seat on the floor and use it when we put Yo Gabba Gabba on for him (mommy's secret weapon when Mc Screamy comes to visit) :P
My son hated it too - we borrowed one from a friend. The only way he'd sleep swaddled was if we left his arms out... which kind of defeats the purpose.

I never use the nursing cover for breastfeeding in public, I'm just discreet or use a bottle of formula instead. I also only used the baby tub twice before we switched to cobathing which was sooo much easier. Then DS has begun bathing on his own in the big tub in the past month, which is even easier.
