Multiples freebie requests

Does anyone have the info and the letter used to send to companies requesting freebies and additional coupons?  Has anyone had any luck with this or is it a waste of time?

I have the letter and addresses on my blog, along with descriptions of what response I got from each company. For me, it was totally worth it, though it would've been even more so if I'd known in advance what I would get from each company! Just keep in mind that I sent my letters in June so some companies' policies may have changed already.
