I take the pillows off in the morning so that the dog can't lay on them....and I also keep an extra flat sheet over everything to keep her hair off the blanket/quilt.

At least once a week I do the whole lot, sheets, duvet cover, pillow cases and blanket that's ontop of the duvet (for our dog who jumps in in the mornings).
I'm a bit OCD. I also use the pledge pet hair removal tool every morning when I make the bed to make sure there is no dog hair - I highly recommend it to any other dog lovers out there.

Every other week, but we do pillow cases every week.
ETA: we both shower at night, so that makes a difference.

I wash the sheets every other week and the pillow cases every week.
We both shower every night and we never touch the bed in day clothes.
