I never have but we also live in a warm climate. But even on the cooler days I don't because it is just too many clothes to take off when changing diapers and such.
I know a lot of moms do a onesie under the sleeper. My LO always gets sweaty when we do that, though, so we either do a onesie with pants or babylegs, or a sleeper, but not both. We haven't had any diaper blowouts or anything, though, and DD hardly ever spits up. If she was a super messy baby, we might do things differently.
I put a onesie under everything she wears. In a sleeper, I just put a onesie underneath, but then I swaddle her for the extra layer at night for warmth.

We do because Grant does not like to be cold at all. However if it is a fleece sleeper, we don't use a onesie. We usually put a fleece sleeper on after baths because #1-he's cold and it is faster, #2- it is really hard to pull a onesie over a tiny arms that have lotion on them. Hope this helps! Oh! And we usually use the long sleeved onesies as well.
I'll only put onesies under 2 piece outfits, or under a dress. Too much work to undress her for changing, otherwise. Right now she's in a long-sleeved onesie and footed pants.

We do white onesies under two piece outfits and the cotton sleepers. We also leave socks on in the cotton sleepers. If DS is wearing a fleece sleeper then he is just in his diaper. My DS has cold feet which is why we keep the socks on and he sometimes has blowouts in the cotton sleepers which is why we have the onesie. He doesn't blowout in the fleece sleepers though oddly enough.

Same here. LO seems to get really warm when she sleeps and being overheated is one of the things they say contributes to SIDS. She has her pjs and is swaddled.
Only if he's wearing a shirt and pants or we're going out and he needs the extra layer. If we're just hanging around the house or going to bed I don't put the onesie on.

Like this weekend, it's getting down into the 20's and 30's. So he's wearing a short-sleeved onesie under his fleece jammies.
But typically, I never put him in an "undershirt."
