Concerned about LH surge before trigger need help please

I had monitoring today (ultrasound only) and after some humming and haahhhing my RE decided that I should trigger tomorrow night.

After I left the appointment I started to get really crampy, like I may ovulate. I took an OPK and it is positive.

I called my RE to see what I should because I don't want to ovulate on my own before tomorrow night and I'm stressing.

Is it too late to trigger if I am already surging on my own? If I don't trigger will only one egg release (I have three follies)?

Thanks ladies.

  • Are they doing bloodwork too?  I had a near positive OPK the day before my final monitoring (I was so sure we'd miss O that we had sex that night just in case) but my bloodwork showed I hadn't yet so I triggered that night.  GL!

    No, just ultrasound.

  • Hmmm - did they do bloodwork yesterday?  They should be following your E2 and LH levels every day at this point.

I would ask the RE to check LH today.  I think you could trigger today if you are indeed surging, but I don't know that as 100% fact.  Some REs will give an antagonist urgently to try to stop a natural LH surge.  So it seems that there might be something you could do if you knew you were surging.

It sounds like this is one of those times that you need to loudly advocate for yourself.  Insist on bloodwork!

    Good luck.

  • Definitely talk to your RE.  I did read somewhere (although I'm not sure if it's accurate) that sometimes cycles where the trigger is timed to the natural LH surge are very successful (i.e. the eggs are really mature).

  • This happened to me on a Clomid/IUI cycle.  I got a positive OPK in the morning and was supposed to trigger that night.  They bumped up my IUI to the next day.  I did still trigger that night, though.  They generally do IUI's the day following a positive OPK or 36 hours after a trigger shot, whichever comes first.  I would definitely call and let them know about the positive OPK and see if you can get your IUI bumped to tomorrow.

  • Thanks ladies! I spoke to my RE. I took the shot today and I'm going in late tomorrow morning for my IUI. That was incredibly stressful!
