concerned about glucose test results

I got a call from my dr's office yesterday telling me my glucose was high on my 1 hour test and they needed to me to come in for the 3 hour test.  I asked what exactly my result was, and the nurse pulled it up and said, "It was 135... Oh, wait.  You don't need to come in.  The lab flagged it because it was high for a non-pregnant person, but since you're pregnant, yours will normally be higher.  Your result is fine."  I said ok, breathed a sigh of relief, and hung up the phone.

I started doing a little reading on the screening test and found that some doctors use 140 as a cutoff, while others use 130.  Obviously, my doctor uses 140.  This made me concerned that I would be considered high according to some doctors. 

I have access to a glucose monitor at work because I teach patients how to use it.  I decided to check my blood sugar 2 hours after I ate lunch yesterday.  I thought surely I would get a good result, which would ease my mind.  I know I didn't eat the best lunch (soup, sandwich, and a cookie), but I really didn't expect my 2 hour number to be 150.  I've taken my 2 hour glucose numerous times before I was pregnant, and it was consistently in the mid-80's to low 90's. 

I have several risk factors for GDM, including being overweight pre-pregnancy (5'3", 155 lbs) and having two 1st degree relatives with Type 2 DM (both parents).  What I would like to do is check my blood glucose more over the next week.  If I continue to get high readings, do you think I'm crazy to request theh 3 hour test?

  • I think you're probably overreacting, but at the same time, there's no harm mentioning your test results to your doctor at your next appointment and getting his/her opinion on the matter.
  • I wouldn't worry about it.  If your OB uses 140 then I would go with that.  My OB uses 130 and I failed the 1 hour with my last pregnancy by 3 points.  If he had used 140 I wouldn't have had to take the 3 hour which would have been really nice.  I think *most* people that fail by a really small amount usually go on the pass the 3 hour anyways. 

     If it makes you feel better then metion it to your doctor at the next appointment, but I really do think you're fine trusting the 140 cutoff.

  • You might be freaking out over nothing but honestly, I would be doing the exact same thing.  As it is, my doc uses 130 as a cutoff and I got a 128 and I was like, "Are you sure I don't need to take the 3 hour?  It's so close."

    If I had access to a glucose machine, I would definitely check it over the next week and then talk to your doc at your next appt.  I hope all your worrying is for nothing!  GL!

  • I'm sure you have nothing to worry about but if it makes you feel better just talk to your doctor about it at your next appointment.

    You 2 hour post meal blood sugar was high, so I would continue to monitor it on your own - fasting blood sugar, and one or two hours after every meal.

    My fasting is supposed to be less than 90, and one hour post meals is supposed to be less than 130. (Some OBs do one hour, some do two...so I'm not sure what the pregnant/two hour number should be other than it should be less than 150.) ;)

    I actually skipped the 3 hour test and went straight to the diabetic educator/GD diet since I have so many risk factors. My one hour GTT was 152.

    This is my second week of monitoring, and I can't get my fasting blood sugar less than 90 (it's usually mid-90s to about 100) so I'm looking at going on bedtime insulin soon.
