Slow cooker chicken recipe

Can someone please give me the recipe for the chicken slow cooker meal?

I know that it has 1 can of corn, cream cheese and the chicken but that is all i can remember. I left the recipe at work and am home today with a chicken DS.

Chicken Breasts (frozen is fine)

1 can corn (not creamed)

1 can black beans

1 jar salsa

Throw it all in the crock pot and put on high for 4 hours (or low for 8hrs), stirring occasionally.  Drop in a brick of cream cheese and let it melt in for about 20 minutes.  Stir and serve over rice.

I just made this last night! LOL. I don't have the actual link to the recipe but this is what I do:

Put frozen chicken breast in bottom of crock pot (I used thawed once too but it definitely came out better yesterday with the frozen). Pour jar of salsa over chicken. Then add can of black beans and bag of frozen corn. Cook on high for 4 hours. Add 8oz cream cheese cook additional half hour. Then I turn off the crock pot, shred the chicken and mix it all together and then let it sit while I cook the rice and serve it over the rice.
