Where to find replacement glider cushions?

DH and I have a glider we got from his grandparents.  It looks similar to this:

I am trying to find replacement cushions for it.  Amazon has some, but it doesn't look like the part that goes over the arm will work with the style we have.  Any ideas?  I don't want to just re-cover the cushions, I rather replace them b/c they are so old.

    I had mine replaced before at a furniture store. I found a similar style where I knew the cushion would work and they were able to order me just the cushion.

    I have the same one as well and we lost the bottom cushion in a move and I really want to find replacements as well.
