I bought one for DD. It was just a regular cake with an extra layer, carved into the shape of a skirt. It had a tub in the middle for the barbie. you could just cut a circle out of each layer and use a paper towel roll wrapped in tin foil.
I bought the Wilton cake mold at Michael's. They have dolls at Michael's that are just from the waist up with a stick in the middle. It's very easy. If you're going to decorate any additional flowers or designs, I always use the Wilton frosting from Michael's too. Betty Crocker/Duncan Heinz frosting is fine for the first layer but doesn't work to put into the decorator bags, it's too soft.
Let the cake cool for 10 minutes after taking out of the oven, then let it finish cooling on a baking rack (or else it won't come out of pan).
If you use a Barbie, you'll need two boxes of cake mix (or double batch if you're doing homemade). Wrap her legs in saran wrap before sticking her in the cake.
