I didn't get a "guest book" but I did get a really cute baby book from Target. Most baby books have a place for shower guests to sign. Babies 1st Furniture had tons of baby books, but I don't remember ever seeing just a guest book. GL
Instead of a guest book my mom got a large picture frame with an extra large matte for guests to sign and write a little message. It has a pic of my boyfriend and I from our maternity photo shoot, and we are going to hang it in the nursery.

I had no guest book for any of my 4 showers. For my DD's shower her hostess had a book with just blank pages that was passed around and people wrote down advice for the new mom and dad.
When I'm a host, what I do is buy a nice photo album with 4x6 slots. Marshall's/Home Goods is my fave place for nice albums at a low price. There's such a variety that I usually end up with something that matches the shower's colors/theme perfectly!
At the shower, we hand out 4x6 card/paper to each guest asking for a message/advice to the mom-to-be.
The nice thing I find about this is that people take their time filling out the cards at their seats before we go around collecting. By the end of the hsower, we've inserted all the cards in the album to give to the mom.
Bonus: the mom can insert photos from the shower in the album. Which means she won't have to store a seperate sign-in book and photo album. All that new baby stuff takes up enough closet space.
