Delayed Vac schedule for 12 months?

Up until now, we have mostly done a standard vaccination schedule (we had a few changes, but she is now current with the CDC rec.).  I am considering doing an alternative schedule for the rest of her shots, but do not have time before her one year appointment to get the Dr. Sears book again (checked out from the library awhile ago).  Does anyone have a copy of the alternative schedule or any suggestions on which ones are most important to delay?

we're following sears sched, and DS just had MMR at his 1 yr checkup. (since this shot is already 3 in 1, they recommend to only do one shot instead of two like when it is Hib or Prevnar or such.

Ditto pp, although we somehow got a bit wonky on everything, and we have chosen to delay the MMR until closer to 2 yr.
