Ok Ladies another blow out what am I doing wrong??

Hi I posted about our first blow out the other day and well it happened again. We are using BG 3.0. It is coming out the front! DD was with DH today and he always puts her diaper on tight. Do you think DD could just have alot of poo all at once and its to much for the diaper so it comes out! LOL. Maybe its not tight enough? DD is breastfed and we give her oatmeal cereal every other day how ever she had a little bit of home made apple ssauce this afternoon before her blow out.  DD normally only has one poo a day. Help me figure out whats wrong. We really like CDing but I hate getting poo every where and all over her. We've been using CD's now for a little over a week. Thank You so much. Oh by the way this is our first blow out with the BG the first was with a FB.

DD weighs 16.5 pounds if that helps and is a little tall for her age.

Is it happening after she sleeps? And does she sleep on her belly?
