I use the Inchbug labels for bottle and used sharpie on the caps but it kept washing off, so I put hot pink hair rubber bands on the caps. I don't date- but I heard painters tape makes a good way to date and remove.
I use Sharpie directly on the bottle for his name and bottle # and masking tape for the date/time. (I provide 3 bottles--to cut out any confusion, I # them 1,2 & 3 so they can easily see the order of use)
Sharpie does wash off in the dishwasher, however. I sometimes wish I wasn't so cheap and would have sprung for some of those name labels or something. I have to re-write his name every few washes.

I use a label maker and plastic labels...or you can just write on them with a sharpie.
