First appt with MFM dr tomorrow - any advice?

DH and I have our first appt with the MFM dr tomorrow.  I have mthfr and am ama (36).  I have looked up on the web some questions to ask (exercise, diet, potential bed rest, etc..) but if any of you can recommend any additional questions I should ask the Dr. I would really appreciate it.

Who will deliver, MFM or OB?  (Are you seeing both?)
How often will you see the MFM? 
Will there be an u/s at all appts?
How many multiple pg's has the MFM dealt with (and how many with your dx?)
c/s vs vaginal
Do they have a nutritionist at their office?  Mine did and they gave sample menus for eating at home or eating out using Dr. Luke's book (which rocks and is a "must have" if you don't have it already).

I'd also ask any other questions you might have already asked your OB -- it is good to know they are on the same page since they should be working together on your pg.  Also how do they communicate?
