Are all of you seeing a MFM before TTC Again?

I'm thinking about making an appt to see a MFM. My regular ob, IMO, is overly optimistic about my TTC again (he is the high risk dr in the practice). I want to know the real deal and be tested for clotting disorders. Also, my Dad has Gilbert's (liver disorder) which is hereditary (there is not test for it). I want to know if that could impact HELLP. Anyway, I just felt like my ob was like "Yay! TTC again anytime after 1 year...hopefully you'll be pg next time I see you" but I honestly, if there is some underlying issue, I probably won't TTC again. I just want the brutal truth, and I'm not sure I'm getting that with my ob...

  • Well, I can tell you about the appt I had with my hemotologist the other day. I was tested for clotting disorders and I am positive for two of them. My doc said that if and when I get pregnant again, I will probably need to be on heprin or something similiar. He said that asprin would not be enough for me. I have yet to visit my OB to discuss in detail about TTC again, but I really want to, even though we aren't planning on it for at least another year. I would be skeptical about a doctor who is so excited without hesitations. My advice (and it sounds like you are already planning this) is to get tested for the clotting disorders. My doc also said that I shouldn't be on the pill because of these disorders because is increases my chance of stroke 2 to 3 times. For now I am going to remain on the pill until we have one more baby, and I'm going to work on losing more weight (yay only 60 pounds to go! :( ).

    I seriously left the hemotologist office with my head spinning. There is so much to do and to prepare for just thinking about TTC. Have you asked your OB what the chances of getting HELLP are? I haven't asked yet, but that will be at the top of my (long) list when I do go see her.

    No, my OB thinks it was a one time thing due to a fibroid in my uterus. I don't agree and think it stems from my progesterone issues but unfortunately there is no way to tell until we try again :(

  • No, just working with my OB, but I didn't have the same issues that you did, and I think the more information you have the better decisions you can make, right? Let us know how it goes!
  • No.  We are just seeing our ob bc our IF issue was PCOS and metformin worked last time so we assume it will again.  My ob has also talked about bedrest earlier and trying to make it to 39 weeks since DS' lungs were underdeveloped at 36-37 weeks.  Mostly she just thinks it was a fluke though but I appreciate that she is being cautious so I feel comfortable just working with her.  I do think though that if I had had more serious Pre-e or any other more serious condition I may do some further digging.  I have done a lot of online research on PCOS and am also going to talk to my doc about a few things I have found online as far as diet and med during pg to help with fetal development and over all health of the pg.

  • Yes, I saw both my OB, MFM, and rhematologist. My course of action for when we try again is very lenghtly. We have developed a plan between all 3 doctors for the course of action.

    I highly recommend seeing one. A regular OB doesn't specialize in high-risk cases, a MFM doesn't specialize in immune issues, and a rheumatologist doesn't specialize in childbirth. However all 3 combined gave me the confidence to get pregnant again.

    Everyone's case is going to be different so seeing everyone and anyone that you need to, will help ease your mind.

  • Yes. my OB told me at my 6w pp appt that if we decided to TTC again, she'd send me to the MFM beforehand, and if I showed up pregnant (I laughed. We had conception issues.) she'd send me straight there and I'd be comanaged between them for my entire pregnancy.
  • we didn't... but we weren't exactly TTC either.  We're just seeing my OB now... she seems to think that things will go much better this time around.  I had BP issues and they've already started me on BP meds this time around.  Also, I just started on progesterone injections which are supposed to help lower the chances of pre-term labor from around 30% (for 2nd or more timers who had a preemie the first time around) all the way down to like 3%... so the odds are looking good so far.  We'll see how it goes though.

  • I don't need to be cleared to TTC, nothing was going on besides it being a triplet pregnancy. My MFM did say he would see me for my next pregnancy though so I will not go to a reg. OB.

  • I did and I'm glad I did.  My OB told me I had a 15-20% chance of having pre-e again and the MFM took a thorough family history and said I have a 60-80% chance.  I think that's a big difference.  Also, the MFM ordered the bloodwork that diagnosed a blood clot disorder which my OB never even thought of.  So...I highly encourage it.
  • Not sure yet.  I guess I will see what my OB says when we discuss TTC, which we won't try until next summer.  If my OB thinks I'm going to be high-risk, then I guess I will see an MFM just to be sure there isn't anything I'm missing.
