we don't have to pay for them, but we have to fill out a form to have the forms filled out, and then they send them in to someone and then a few days later we have to come get them.
nope never had to pay. However if we need paper copies of their records we have to pay...cant remember how much though.

Our pedi's office offers you one of their forms at the regular visits free of charge. I just make copies, b/c if you request them any other time, there is a fee.
I'm not sure about needing specific forms for preschools, etc. as the only places we've needed them for R have accepted the pedi's form.
i visit doctor's offices for a living (drug rep) and can say that about 80% of the offices I visit have a policy posted that there is a charge for filling out forms.... the other 20% probably about 15% of them do charge, but just don't have a sign up.
Time = money, esp in a doctor's office. It's quite common. Same goes for asking for copies of your records. Takes time to make them, costs money to print them out/copy/paper, etc.
Yes. ?It used to be a $5 fee, and I've noticed the sign now says it's $10. ?I understand them charging a fee, though. ?It takes time to fill out forms, look up vax records, etc. ?All of the public and private schools around here require a specific form, and they have to be done every year, so that would be a lot of work for free if they didn't charge.
