IVF #1 Baseline today - and AFC Question

Today I had my baseline appointment for our first IVF.  Haven't gotten blood results yet, but the u/s went just fine.  No cysts, "quiet" ovaries, fully suppressed.  My antral follicle count was 18 - it wasn't my normal Dr. and I couldn't get a sense of whether he thought that was good.  Dr. Google tells me it is good, so I am happy with that.  Just curious though - has your AFC been predictive of the number of follicles at retrieval?  I know maturity and viability are a whole other issue, just curious if your AFC was close to the number of eggs (mature or otherwise) retrieved.  Thanks!

  • I have found that things vary from cycle to cycle. My original AFC, back in the day, was 16. My first IVF we got 14 eggs (12 mature, 8 fert.). A couple months later, IVF #2, we got 10 eggs (8 mature, 7 fert.). 2.5 years later, AFC was at 9, IVF #3 we got 17 eggs, (10 mature, 6 fert). IVF #4 we got 10 eggs (9 mature, 5 fert, ugh).

    So as you can see it's all over the place. My Dr's have told me that your ovaries react differently every cycle, which is true, in my experience. This last cycle my left ovary only had 2 follies, and all other cycles that is my rock star one with 8 or so.  Also, once they can gauge how you respond (at least at my RE's office) they pay less attention to AFC and more to past response. They barely even count my AFC anymore.

     18 is a great count though. Best of luck!

  • I don't know about your case, but in my case I am a poor resonder with low AFC.

    This time my AFC was 7, 3 follies at ER, 2 mature.

    So, if you go by those numbers, then you are in GREAT shape.  You'll have a good 6-8 mature eggs. 

    Other women, get more follies to poke through with stimulation.  So, if you are at 18 now, you could be at 10-20 mature eggs for retrieval.

    Either way, best of luck....and only time will tell.

  • My AFC a week ago was 14 at baseline- and yesterday after 3 days of stims I had 16 measurable follies.

    So 18 is good- AFC is good way to judge- what may happen- but its not the end all be all.

    My AFC was always close to the number of mature eggs I got when I did a heavier stim protocol.  I tended to overstim, but when we tried the low and slow approach I got fewer mature eggs.  So - if my AFC was around 20, I got close to 20 mature eggs when we pushed my ovaries, and around 20 eggs total with only 11 or so being mature when we were more gentle.

  • Thanks for the input ladies! I know everyone is different, it's just so hard not knowing what to expect.
