what papers do you need to sign up dd for mdo??

I am going to sign dd for the local MDO program.. And I have her info ready to go.. just want to see if I am missing anything TIA!!

    I needed to submit an application, her vaccination records and her birth certificate in addition to the fees and deposits.

    Oh, and there was also a detailed questionnaire about her history/interests, etc.
