Microdose lupron flare protocol? (SAIFW)

It sounds like I might actually be doing the microdose lupron flare protocol for IVF #1, instead of the antagonist.

Anyone have any experience with this?  What was your injection schedule like?  Response?

IVF #1 = BFP on 5/2009
FET #1 = BFP on 5/2011

Re: Microdose lupron flare protocol? (SAIFW)

  • I will be doing this protocol for my IVF too. I started BCP last night. Instead of taking BCP, then injecting Lupron, then starting stims, you'll actually take the BCP and then take very tiny doses of Lupron at the same time as the stims. My IVF nurse said that I'll only need 25 units of Lupron total. I'm also going to be on a high dosage of Gonal-F, although I'm not sure yet what dosage I'll be taking. It's the protocol for poor responders.

Edited to add: My RE told me that something about the tiny dose of Lupron causes the FSH in the body to flare up, providing a better response to the Gonal-F or Follistim. Poor responders can produce more eggs with that small burst of FSH.

  • I've done MDL twice and it's not a very good protocol for me. First IVF was canceled due to poor response. Second IVF was the same protocol as part of a research study for low responders. Both protocols used 10 Lupron 2x/day plus 525 Gonal-F and 75-Menopur 1x/day.

For my second IVF I ended up with two follicles and we went for it. Got 2 eggs and one good embryo and am waiting for my beta on Monday (I'm not hopeful because I have no symptoms whatsoever).

    Next time I'm trying antagonist.

Forty-something TTC since 12/2007 3 failed IVFs DE cycle #1: BFP then D&E at 12 weeks due to neural tube defect DE cycle #2: Chemical FET #1: BFN Lining issues, pursuing adoption
  • I did MDL protocol for this IVF.  For me I did 4 injections a day, 2 in the AM, 2 in the PM.  20 U of Lupron and 300 IU of Bravelle for each time.  Plus I was taking dexamethesone (oral) and DHEA which was started two months before this cycle.

Compared to my first IVF, which was 450 IU of Bravelle once a day, a couple days of Menopur and cetrotide - The MDL protocol produced much better results for me.

IVF #1 - 6 eggs retreived, all mature, 5 fertilized, none were of the top quality, transferred 3 at 3dt, BFN, none to freeze

IVF #2 (MDL) - 20 eggs retrieved, 18 matured, 15 fertilized, all survived to a 5dt, transferred two blasts and 5 made it to freeze.  I'll tell you Monday if it worked or not.

TTC since May 2006. After 3 failed Clomid cycles, 2 failed Injectibles/IUIs, 2 failed IVFs and 1 failed FET, we moved on to adoption!

  • I did it for IVF's #2 and 3 and, for me, it was pretty much a disaster protocol, but, I know others have had great success with it!  My first time doing it I only got 3 follies and was downgraded to an IUI.  My second time doing it I got 2 follies, but, my estrogen started dropping and the cycle was cancelled outright.
  • I did MDL for my most recent IVF.  I started with 20 unit of diluted Lupron in the morning and 20 in the evening for 2 days, then added Follistem and Repronex in the evening.  After having 2 canceled cycles, I guess you could say this one was a success in that we at least got to go through the ER & ET.  They retrieved 14 eggs and 8 fertilized.  We made it to a 5dt, but on had one full blast - excellent quality.  We transfered that one and none of the others made it to freeze.  We got a bfp, but found out at our 7 week u/s that it was a blighted ovum.

Good luck.  I hope it works for you!
