I read your post about it yesterday and I was going to respond, so I don't even know if you got this info or not or if this is the route/information you were looking for, but when I had my test done, my RE did a "mock" medicated cycle and then had me go in around the estimated ET time and she took the uterine sample in her office and put it in the little specimen jar thing, and I had to bring in some paperwork with me, she might have had to fill something out on it, I can't remember, and then they gave me the specimen and I shipped it to the lab that does the testing myself along with the payment.
I will caution though that it is a little expensive to have done, and they don't have great data showing the diagnosis and treatment is beneficial. I know that there are studies that specifically link the lack of protiens and implantation issues with endo (not sure if you have this or not). Anyways, if it is really something you want to pursue, maybe your RE getting the sample and you taking care of the rest is something you can talk over with your RE? I think the only place that actually does the testing is Adeza out in California (or at least it was when I had my testing done).
Thanks, that was all very helpful! I was pretty sure it got sent away. A lap last fall found Stage 1 endo which they removed. I just really want to pursue this before using our 2 frozen embryos.
