I gave her a small travel pillow (about 14x20) around 11 months old. Sometimes she sleeps on it and sometimes she sleeps away from it, but she's done fine with it.
we started early maybe 9 months. he would fall asleep in our arms but would wake up if we laid him flat, putting him up on the pillow he would stay asleep. he has never had a problem.
I put it in his crib about 2-3 weeks ago. I don't think he's really using it. I don't know for sure b/c once he goes down, I don't go in there. But, when I get him in the morning (which is only on the weekends since DH does the morning routine.....), he is not laying near his pillow!
Re: How old for a pillow?
We put one in DD's crib when she was just over a year old. I tried it b/c she had a cold and I wanted to elevate her head a little bit.
She's actually been sleeping a lot better since I let her sleep with a pillow.
I just got one for DS through www.mrbobblesblankets.com (its a toddler pillow....)

I think eventually he will use it!
