I would wait until the big u/s when you find out what your having. If your not going to find the sex out then you could start to register now. Babies R' us is a good one to register. I would stick to one store to register so everyone can buy off one and you will get your register list more complete.
I don't think it's too early to start, at least researching what brands you want and such. I did most of my registering after my big ultrasound at 20 weeks so I knew to pick "girl stuff".
I registered at Babies R Us. I don't particularly like their return policy and sometimes their customer service left something to be desired, but it was okay overall.
You could definitely start researching what you want now, and creating a registry won't hurt either. No one will likely see it for a while, anyway. AND you can always change it
We registered at BRU and Target (easy location for our families). Target was okay, but online stock differs from the store stock and each store even differs. So if we registered in Redmond and family went to Woodinville, they may not find what we want. BRU never had the stock issue, but I HATED going to the store! It is always a madhouse of hormonal preggos!

I registered on BRU online and never stepped foot in the store. It was so easy and done in the comfort of my own home. I would go back and add/update etc. for weeks. It really made everything very simple.
I registered after we found out we were having a girl which was at 22 weeks. (We found out late) But, if I would have known earlier I totally would have done it. We only registered at Babies R Us, but there is also Target, Pottery Barn Kids......etc.
It's a good idea to start now because I found it to be exhausting to go around the different stores, and even looking online got tedious. Plus you can always change things.
My favorite registry is Wishpot. It's online, you can choose from several different stores, add notes, have people contribute money towards larger ticket items, and the pictures are pretty cool too. So far, of my two online registries, that's the one everyone used.
