I just had one (finally at 23 weeks) and they just did it as part of the external US - it was part of my anatomy scan (yes, it was really late).
I have been feeling a ton of pressure was really worried about it. It was surprisingly at 5. I assume that if it was shortened, they would have wanted to get a more accurate reading and would have done the internal.
I have had multiple u/s techs tell me that they are not supposed to quote cervical length from an abdominal u/s. She'll say, it looks long, but we have to do an internal/vaginal u/s to confirm and measure. Therefore I'm not sure how much stock I'd put into a measurement from an abdominal u/s.
I only had 1 internal u/s to measure cervix when the babies were too big to get a read thru abdominal u/s. All my other cervical checks were abdominal or manual exam - and I had it checked at every appt.

It should just be part of the u/s
it's a vaginal u/s (with an u/s wand), and they just look at the screen and measure the length of your cervix in cm. mine has been 3.7-4cm thus far.

They can check it with the ultrasound on your belly or an internal. Mine always does an internal just to get a more accurate measurement.
