11 week ultrasound could this be twins?

I was wondering if anyone was also seeing an extra sac on this ultrasound or just the placenta my midwife said she wasn’t a very good sonographer and couldn’t give any explanation she said the heartbeat was hiding behind mine and struggled to find the initial heartbeat my actual ultrasound isn’t for another 2 weeks and was really confused by the sac to the right
