Day 3 Test Confusion

Hopefully this is the right place to post this! I'm so tired from constant research and iust want to learn from others who know more than me.

I'm 31 years old, and my husband and I have been not trying but not preventing for over 7 years. We were on the fence about having kids for a long time, so we didn't really care that I wasn't getting pregnant, though of course I thought it was weird.

Periods suddenly became very irregular at the end of 2017. Most cycles are between 28 and 40 days, but I'll occasionally go 60, 70, or 80 days between cycles.

Just started looking deeper into my fertility and completed all my baseline tests, but since the doctor won't be able to see me for a while, l'm looking for some opinions.

  • HSG test was normal, everything looked good.
  • I did my "Day 3" tests on Day 2, which I was told was fine. Antral Follicle Count ultrasound was good. 8 follicles on one side, 10 on the other, and ovary sizes were good.
  • Now for the blood work, which kinda has me baffled:

Thyroid:

T4: 8.5 (normal)

T3 Uptake: 21 (low)

Free Thyroxine Index: 1.8 (low end of normal)

TSH: 2.42

Other hormones:

Anti-Mullerian Hormone (AMH): 1.87 ng/mI

Hemoglobin Alc: 5.7 (prediabetes--l'm aware of this and working hard with diet and lifestyle to reverse)

LH: 6.5 mlU/mL

FSH: 9.5 mlU/mL

Estradiol: <5.0

Testosterone, Total, LC/MS: 8.1 (low)

This is most of what they looked for on day 3, but for some background context, spent a couple years treating possible hypothyroidism, but saw no change in my cycles or fertility, so I came off the drugs as they didn't seem necessary. It was kind of a "try it and see" approach. I also have insulin resistance and am working hard to reverse it through lifestyle change. My DHEAS has been tested in the past and is always VERY low.

It's tough to find anyone with my combination of high and low numbers online, so hoping someone has some perspective. TIA!!

It seems you are forging ahead with your tests.  But remember it takes two to tango.  Your husband also needs to get his sperm count.  What is important is to address your diabetes and insulin resistance.  Your hypothyroidism might complicate your IVF process so maybe continue with your medications?  Are those prescribed by the specialist?  Either way, continue trying naturally with DH.  Who knows, you might get a miracle baby.
