Looking for advice after 2 failed FET, TW mention of living child

My second FET just failed. My husband and I are undergoing IVF to avoid a genetic hereditary disorder and find a donor match to cure my daughter of the same disorder. I'm 34 and don't have a history of infertility. We did genetic counseling, a mock transfer, and sperm analysis - everything came back fine. I had one missed miscarriage 3.5 years ago at 10 weeks followed by a normal pregnancy 3 months later that resulted in my daughter. I did an egg retrieval in May and was very fortunate to find 5 good quality euploid embryos that are matches for my daughter.

For my first transfer, I took Endometrin suppositories 3x day and one Ovidrel trigger 5 days before transfer, metrogel the night before transfer and 200 mg progesterone 2 hours prior. For my second transfer, I took Estrace 2mg orally 3x/day, Estrace 0.5mg vaginally 1x/day, started PIO shots every morning 5 days prior, metrogel the night before transfer and 200 mg progesterone 2 hours prior. I also did acupuncture. My lining took a couple extra days to thicken, but otherwise all of my levels were normal according to my doctor. There was no found reason for the failed transfers, which is so frustrating.

I've spent $45K so far and barely have enough savings to do one more transfer - I need advice. My doctor said I could take a cycle off and do testing such as an ERA, X-ray of fallopian tubes, a monitored medicated cycle to see if the timing of transfer is right, check for endometriosis, or test for chronic inflammation. But there has been no indication to do the testing other than the failed transfers. My doctor's recommendation is to skip the tests and do another transfer adding some medications that would combat possible inflammation or treating things that could potentially come back based on the tests.

She recommends doing a 3rd transfer beginning with Letrozole, no additional estrogen, progesterone with suppositories or PIO (which one is my choice), take progesterone at night to give me an additional 12 hours of progesterone before transfer, trigger ovidrel, baby aspirin 1x/day to improve blood flow, benadryl 1x/day at night to combat inflammation, Medrol low dose steroid to relieve inflammation for 10 days, and doxycycline for 2 weeks to combat any potential hidden infections.

I don't know what to do. Throughout this IVF process, I've followed doctors orders and haven't had much input on what meds to take, and now my doctor is asking what I want to do, a la cart style. I'm so overwhelmed. I do have some inflammatory health concerns, but they're minor: seasonal allergies, eczema, and acid reflux. I hate the idea of taking steroids and doxycycline makes me feel like crap but I'll do anything to make this work.

Does anyone have insight into the suggested protocol? Success after 2 failed FET with tested embryos? Thank you to anyone who read this far. I feel guilty complaining when I know so many of you have been through far worse disappointment. Wishing everyone success on what I have found to be the hardest journey thus far.

  • I don't know enough about meds and protocols to offer sound advice. However, you can have a consult with a different doctor for more input (you can check SART for success rates to compare clinics). You might also want to consider a surrogate as another (but quite expensive) option.

  • @anon84027 I agree with @00kim00. It would be best to seek a second opinion with another fertility doctor. I found the Fertility IQ a helpful website to find a doctor who could be a good fit.
