Any twin moms have early pregnancy scans and the second baby was missed? I’ve had extreme symptoms and noticed a “spot” on my early scan but the doctor didn’t mention it

The doctor would have told you if this "spot " was something.  Please trust the expert eyes of a trained medical professional.  If you don't trust him/her, find someone you do.  The doctor didn't mention it, because its nothing to be noted. 

Extreme symptoms mean NOTHING.  Some people have horrid symptoms, some have 1, only a missed period.
