How are things going?: Tw loss* currently waiting for my Hcg to get low enough for things to start with this loss.

So my re seems to think that my issues are from chromosomal problems. I’m having a hard time understanding how that is the conclusion she’s come to. From things I’ve read, my amh would give some clue as to my eggs being poor quality. I know it shows my ovarian reserve as well, but from 2017, my amh was 3.58. Isn’t that considered pretty good? I’m 33. Should I really be concerned about poor egg quality??? (If I need to edit any of this please let me know.)

ETA: isn’t there a way to test for progesterone deficiency? My re claims any testing wouldn’t give us an idea because of how our body releases it, but how are people diagnosed? Maybe I’m just missing something. I got it tested with a different dr back in 2017 and my 21 day came back at 0.8. That seems a little low to me.

How are things going?:   fine, so far.  Just took my third dose of letrozole, so ticking right along.

Any questions?:  I know I've seen this discussed before, but couldn't make the search function find it this morning.  For those of yall who have taken letrozole/femara, did you O earlier?  I usually O around day 18-20 (if I ovulate at all), and I'm all the sudden nervous it's going to happen way earlier this cycle!

What are you doing this cycle? (Testing? Treatment?): Just tracking my bbt so I know when AF should come, then the plan is BC to pause my cycle, Saline Sono, Stop BC, back to medicated IUI

How are things going?: My hcg came back last week at a 3, so essentially negative. Now just waiting on AF. I feel so impatient. I just want to get back to it. But on the plus side I am pretty sure I ovulated a few days ago.

But I am also having a hard time accepting that we are going to do the same thing but expect a different result. My RE said we shouldn't change protocol at all. *TW*loss ,And I get it, the loss was probably a chromosome issue. But my brain has a hard time wrapping around the idea that what we did worked even though we lost the baby.

@lcking82 the first round of letrozole I took for IUI #1 I O'ed around my usual time of CD19 or so. IUI #2 last month I O'ed around CD13-14. Idk if that'll be the case for this 3rd IUI, but we'll see. I think it varies for people!

@kbrown2385 my understanding of AMH tells me that 3.58 is a perfectly good number for your age! I think less than 2 or 3 is when there's concern.

What are you doing this cycle? (Testing? Treatment?): IUI #3 with the same protocol - letrozole, trigger, and progesterone suppositories

How are things going?: The TTC is going fine. Personal life, that's a bit of a mess. No time to chill! Look forward to things slowing down by the end of the week

Any questions?: I've heard your chances of success increase with each IUI, but what is that based off of? Why is that the case?

@hanshotfirst77 saaame. I briefly considered asking about injectables for IUI #3 for more possible eggs instead of the letrozole which only gives me 1 large follicle. I feel like we're just going through the usual motions to eventually do IVF. But I guess for me, since I've never had any losses and all things look good/normal for me, changes to protocol wouldn't give us any better odds.

@kbrown2385 I don't know the answers to your questions, but it sounds to me like low egg quality might be your RE's default diagnosis when she/he has no idea?

@hanshotfirst77 FX it truly was a chromosomal issue and your treatment plan works next time.

What are you doing this cycle? (Testing? Treatment?): One more hCG test to go, but it was at 7 last time and my period started, so I should be at zero when I test next week.

How are things going?: Things are good. This is my last cycle on the bench and the  the "exciting"? testing starts on CD 3 of next cycle.

Any questions?: My understanding is there isn't really a way to measure egg quality; assumptions are made from quantity. Is that correct?

GTKY: what is your favorite dessert to eat and/or to make? Anything lemon. Probably lemon bars, lemon trifle, and lemon meringue pie, in that order. Followed by all crisps and cobblers.

@lcking82 with Femara I ovulated earlier than usual. I'm typically a cd 18-20ish , and my medicated IUI I was a cd 13.

@inthewoods23 I thought I heard something that they go up with each IUI 1-6, but I am not sure why that is the case. I'm curious of that too.

@kbrown2385 Yes you would do your 21 day progesterone test but if they told you it was normal then that should be okay. However, mine always tests normal and yet I start spotting early hence my LP defect. I was put on progesterone and it stopped the spotting. Something to look into if you're worried! 
@lcking82 I was a late ovulator like you and femara did have my ovulating at CD14-CD16!
@hanshotfirst77 I can imagine how frustrating it is to have to wait. Hang in there!
@inthewoods23 I think they say that because the more you do something, the more likely you are to have success. I'm not sure if the chances increase per time, but maybe if you're body is getting adjusted to the meds and put on a normal cycle?
@ruby696 Yes, not really a way to measure quality until you do a retrieval. I wish there was an easier way for them to determine quality.

How are things going?: Shots are fine.. easy peasy. I had my first follicle scan yesterday after 3 days of stims and ended up crying the whole 2 hour drive home. Only saw 7 follicles and 3 of those were measurable. I go back tomorrow, so I'm just hoping things look better then. If not, this could all be for nothing. I also think the RE could've been much better with his delivery. I'm not even sure how concerned I should be, he just really sounded negative and that cancellation was highly possible.

@kbrown2385 I am so sorry for your loss. I was so sad to see your update on last weeks thread. I believe bloodwork should be able to show what your progesterone is at. Whenever I go in for blood, whether beta or to check estrogen, they always check progesterone too. I agree with @inthewoods23, amh at 3.58 is good for your age! I hope you get the answers you need soon 
@inthewoods23 My RE said the same thing, chances increase the more you do it but I couldnt wrap my mind around the logic. I feel like you have the same chance of success with each try but I'm not a doctor lol. Keeping my fx for this cycle!

How are things going?: Beta was negative on Friday so looks like Ill be around these parts with you lovely ladies for a little longer. I took a test on Thursday morning which was a bfn. I was able to cry and get everything out early. Luckily since we were in the ER with DH grandpa when the call came so I was able to keep my composure around everyone. Waiting for AF (started spotting this morning) to have baseline u/s and blood.

Any questions?: Has anyone done back to back FET? RE said thats the plan as long as baseline u/s and blood look okay.

GTKY: what is your favorite dessert to eat and/or to make? Cupcakes and cookies. Being no sugar right now is killing me ahhh

@ruby696 well she can default to that all she wants, I’m not accepting it until we exhaust all other options. I’m not sure she’s ready for me. 

@kobrien827 I’m not sure what my drs problem is testing my progesterone is. I think I’m going to ask for updated testing for that.

@kbrown2385 that progesterone # does seem a bit low but I'm not an expert by any means. That's kind of strange that your RE jumped to that conclusion without really explaining where that came from. 

@lcking82 hopefully it doesn't make you O super early! Not that it's the same drug, but I O'd at the same time as normal on Clomid (I usually O a little early though)

@hanshotfirst77 hopefully AF makes her arrival quickly. I would feel the same as you about changing the protocol.

@ruby696 really hoping your HCG is down to 0 next week! What kind of testing are you doing? To my knowledge there isn't anything that measures quality, only quantity.

How are things going?: not great. I've been really down the last week or so about TTC and life in general so I've just been a mopey mess. I feel sorry for DH because he feels helpless...but so do I.

GTKY: what is your favorite dessert to eat and/or to make? oh man, I love dessert. Eat: those brownies with hot fudge in the middle with vanilla ice cream (I'm sure there's a more eloquent name for this). Make: I'm bad at baking but can make some mean funfetti cupcakes.

@kobrien827 sorry for the BFN 😢 nobody has explained any kind of logic to me for these IUIs and as an engineer, I just really want to understand! Everything is all fine physically on my end so I think a lot of our problems stem from DH's low morph and maybe we just have bad timing. I keep wondering if his sperm can even penetrate my eggs! What if that's the issue this whole time? Would the usual tests even show that? Sigh.

@b_1029 hang in there! April will be here before you know it! I think what you're thinking of is like, brownie a la mode?

I apparently make some of the best brownies according to DH's family. There's nothing special about them because they're from a box lol. But I bake them just right so they're nice and chewy inside and not cakey.

@b_1029 I'm sorry you're having a hard time right now. That first RE appointment was a huge help to me. Hoping April comes soon for you.

Also, I'll have an US to count follicles and labs for FSH and estriodol. And probably clomid. I cannot wait to be doing something again. I'm over being benched.

Status (WTO/TWW/TTA): WTO/benched DH lost his job so we are just waiting till the home is more stable.

What are you doing this cycle? (Testing? Treatment?): just tracking my cycle this go around. It seems 36 days is a new normal for me. It used to be 30. I think DH is actually continuing on his supplement I got him! We ran out of coffee and I purposefully did not buy him more cause I want him off caffeine a bit more. The sodas will be tricky though. 

How are things going?: annoyed at my cycles being longer, but can’t complain too much cause it would certainly be worse and I’m still in normalish range.

Any questions?: Anyone else have issues with getting DH to discuss TTC? 

GTKY: what is your favorite dessert to eat and/or to make? To eat: molten lava cake. To make: Apple pie. It’s so tedious but I always love the end result.

@madcouplewithabox I think discussing TTC will always be difficult for guys, especially when the IF is MF related. I think opting for decaf coffee is okay right? And my RE wasn't concerned about us having 1-2 cans of soda a day since their caffeine content is a lot less compared to coffee. Should probably still cut soda out with all the sugar...

@crookshanks10 I hope stim is going well for you! Dont get down on the number just yet. My RE said in early stim, they are still small and sometimes cant see them all. My last u/s showed 13 follies and they retrieved 23 two days later. Stay positive my friend!
@kbrown2385 I hope you can get some answers about progesterone. If they are taking blood, definitely ask about adding that test. 
@inthewoods23 DH had 0% morph but RE said even deformed sperm can penetrate eggs so morph isnt a huge deal, at least to her and the urologist we saw too. Keeping my fx for you!

It's hard to not think about all the things that are preventing us from TTC. Poor DH after my last failed IUI kept saying "what if we can't reproduce?" 😢 Breaks my heart. We're not completely at the end of all options so I don't want him to be so down yet!

Can a lurker ask a question?  Although I need to probably face reality and just accept that I probably belong here now.  I apologize for the novel.

TW I used clomid about 2 years ago and got pregnant with my molar pregnancy.  It was my second monitored cycle with my obgyn.  I had stopped ovulating, theory was due to the stress from my brothers suicide.

Once I was released to try again a few months after chemo, my obgyn said to try for 3 cycles, and if I wasn’t pregnant, to see call him and I assumed start clomid again.  Then I was benched for a few cycles due to now cancer scares.  We started trying again around September, my cycles have been relatively regular.  One I question if I did actually O, but I had surgery and I think they messed my cycle up.  I did get pregnant in December and had a CP.  I have since had 2 regular cycles.  I see my obgyn next month and plan to ask then what the next step is.   So, all that, my actual question is, Is there any point to clomid if my cycles are regular? Or should I just call and get scheduled to see an RE?

@holly321 my cycles are regular and I was on Clomid. My doctor said to "strengthen ovulation". I was skeptical too since I ovulate on my own.

@holly321 just echoing what @b_1029 said. When I had my IUI I couple years ago it was to...
control and strengthen ovulation. Apparently better developed eggs are part of it too.  Clomid combined with trigger helped time things for the IUI. I did Clomid with an OB. From what I understand, it depends on your OB. Mine did tons of US while I was on Clomid to monitor stimulation. If your OB wants to do Clomid but does not monitor you I would switch to a diff OB or an RE. My IUI was mostly paid out of pocket. Apparently once you are clinically classified with IF it changes the coding doctors have to use when filing items with insurance. My scans, because of they way they were classified with my OB were almost completely covered, the IUI done with a different clinic was not and out of pocket. 

It just depends really. I would honestly go to an RE in your shoes. They might still do clomid but it would be monitored more closely and they would probably do some different testing to give you some more answers to your specific situation.

@holly321 I still haven’t faced reality that i belong here... 
FWIW with your history I would certainly look to go to an RE who has all the ART tools at their disposal, from clomid onwards and hopefully can advise you what they think is best but also offer whatever you’re ready for.
I echo what was said above, clomid or letrozole or any thing else should be used with monitoring as these are what lead to higher order multiples, or at best you might do months when it’s not right for you. And yes they can help “strengthen” ovulation if your hypothalamus has a tendency to be a bit scared of working at full power.
A good RE might want to do a saline soon or Hysteroscopy, especially after your recent CP. A check for endometritis might not hurt.

@crookshanks10 after three days of stims is still very early. Did you have any suppression before?
Whats your protocol?
Some people start a bit slower and some cycles take a few days longer than others - I stimmend with a deviation of 8 days between fastest and slowest!
Plus what some technicians and clinics will measure, others won’t
If you need to let off steam you know where I am - I started stims 10 times, made 7 retrievals and was a one woman protocol testing machine!

@ruby696 yes without an egg retrieval you can’t start to know if egg quality is the issue or not

@inthewoods23 over the first three IUIs there is a cumulative increase in success rates, but that declines after the third. Though how you really understand and interpret the statistics is quite crucial... so the first IUI has a fairly low success rate (let’s say 20% though studies do differ a bit and the next numbers are also variable). The second has 25% and the third 30%. This means 30% of couples have success by their 3rd IUI - but that typically includes those couples who also had success at first and second. Looking at it the other way, 70% aren’t successful with 3 IUIs... After three IUIs there is only a 1-2% increased chance for each successive one. But in the end %s only matter to a population - you will either be one with success or you won’t. There’s no 30% success.

@kbrown2385 AMH tells you nothing about egg quality in general. Unless you have low AMH due to a genetic reason or if you have very high AMH it sometimes comes with low quality. But most low AMH is in AMA ladies who by nature have less chromosomally normal eggs due to age related quality. As per above to @ruby696 the only way to really see if you have egg quality issues is to do IVF and see if you have fertilization issues, or if embryos struggle to day 3 (which is more about the energy in the mitochondria of the eggs) or if you have very low rates of PGS normals. Otherwise it’s just speculation

Monitoring this morning. My lining is already 14mm on lowest dose estrogen. But it might be too thick. Let’s see.
Estrogen and cetrotide we’re kicking my butt but hope that improves now I can drop the cetrotide.

@emmasemm I think I get what you mean. Each individual IUI itself doesn't have a higher chance of working, it's just that some will have success at the 2nd and others will have success at the 3rd.

It's similar to the percentages they give for couples TTC. Like 95% of couples conceive within a year, but that doesn't mean you have a 95% chance of conceiving at month 12.

@holly321 I ovulate regularly and my RE is going to have me on clomid when I'm off the bench. I'm 38, so she wants to maximize the number of fully mature/hopefully good eggs available to ovulate. My RE isn't super worried about multiples because I guess some people are more prone to twins, but all my losses have been singletons. So that makes me less likely to have multiples with clomid? IDK.

@hanshotfirst77 ah with the “77” at the end of your SN I wasn’t sure if you were 41/2... but at 33 poor egg quality is not common so weird they would jump to that conclusion. Depending on which study you look at you should still have more than 50% of your eggs with normal chromosomal makeup unless you have any genetic issues yourself...

@emmasemm yeah I picked 77 for the year Star Wars came out lol. I was born in 85. But yeah my RE doesn’t think I have any reason to believe it was anything but a chromosome thing. I guess I just have a hard time wrapping my head around that if we did everything right and had the loss that we’ll just do the same thing again. But logically I know there’s no reason to change things up yet.

How are things going?: **TW** just had a D&C a little over a week ago. So I'm still a mess sometimes. Grief comes and goes in waves. But I'm lucky I have a lot of support from DH and family. *End TW**

Any questions?: I see that many of you have had your progesterone tested. Should I request that? I do have a short cycle (25 days). Doc never ordered it.

Im so sorry everyone I've been MIA. I got my FSH back and I've just been really stunned it was 112, however I've still not had a period since November even after provera.

What are you doing this cycle? (Testing? Treatment?): Took Prometrium to induce a withdrawl bleed and so far have had one day of brown spotting yesterday. Then onto femara day 3-7

How are things going?: Hoping to get a little more of a "flo" with stopping the progesterone, but so far not looking like it. My doctor said anything counts so today might be CD2?

Any questions?: I'm just worried I'll jump the gun and take my rx too early. I'm supposed to start on CD3, but what if my spotting turns into a flow later in the week and I've messed it up?

@lcking82 Glad to hear things are moving ahead smoothly so far for this cycle. Thank you for you words on the last weeks thread. I really appreciate your input on my FIL. He was in a tough head space after that visit, but is doing a bit better now.

@crookshanks10 Oh friend, please try not get concerned with follicle count right now. I know in my cycle I was distraught when I went in day 6 (I think) and they only saw 5 and I was so depressed. Went back in two days later and we were up at 16. Right now it is too early to get focused on numbers. Chin up, you got this!

@madcouplewithabox Oh man, there is no way I could get MH off of caffeine. You will have to give me some tips lol. Glad that he is still keeping up with supplements. 

@emmasemm That is a great measurement on your lining. What would they consider to be "too" thick? I guess we are always so focused on it getting thick enough, I never thought of the opposite being an issue too.

How are things going?: We are doing ok everything considered. The stress is there with my FIL and what he is going through. So we have been distracted and have not focused on ttc, which is both good and bad. Just taking it all one day at a time right now.

@kbrown2385 I am so sorry you don't have any real answers.  From what I understand about progesterone levels it has to be done at the right time to get an accurate result.  For example was your 21-day test done with a regular cycle when you knew when ovulation occurred?  I know some REs just say come in on CD21 but others want you to come in a set number of days past ovulation, which from what I've read is more accurate.

@lcking82 Unfortunately I did not ovulate at all with the letrezole at any dose.  Are they monitoring you for this cycle?

@hanshotfirst77 I get your concern about not changing the protocol.  Did your RE give a reason why they thought it was a chromosomal issue?  I'm just curious.  **TW** I'm still waiting for the results of my D&C but my RE hasn't suggested a reason**

@inthewoods23 When I did my IUI with injectables I still only got one follicle so I don't know if you always get more, but I could be wrong.  I do know they stressed to me many times that four or more follicles would result in them cancelling the procedure so I guess one is better than no IUI at all.

@crookshanks10 I really understand the frustration with only having a few follicles.  When I did my IUI with injectables (granted it was a smaller dose) I had to do 12 days of injections before I got one follicle.  FX next US shows more follies!

@holly321 I'm just going to echo what others have said that I think an RE appointment would be appropriate given your history.  FX you can get in quick and get some answers!

@maserrano714 I'm sorry about your loss.  Make sure to take care of yourself.

@daisy0322 I never start my period after provera or after stopping my progesterone so you're not alone.  What day of your cycle was your FSH taken?  I believe it's supposed to be drawn on CD3 so that might make some difference.  Have you tried calling your RE?

@flinstone-2 I never had a bleed whenever I stopped my prometrium but I'm kinda weird I guess.  I would ask your RE what counts as CD1 because my office always says full flow before noon but every office is different.

@nitnat007 It is completely understandable with everything you have going on to be stressed and not focused on TTC.  Are there any updates on his condition or treatment plan?
