If your doctor thinks you need to control your weight gain they should be able to provide some guidance on how to do so. Maybe your doctor can refer you to a dietician.

If you ask her, and she is really concerned she will get you the resources you need to make the changes she wants to see. Give the office a call and ask, it can’t hurt.

Interesting that you edited your original post to leave out all of the personal accountability stuff (your large portions, not being active, etc). I didn’t respond at first because @meggyme had a great answer and suggestion. But honestly, part of it you already know, and then dirty deleted. Cut your portions back, make healthier decisions, and move as much as your body will allow. And referral to a dietician is a good idea. Nothing is stopping you from calling and asking for a referral. Plus one to all of the above. There isn't an easy way to stop gaining without making any sacrifices. If there was an easy way, don't you think people would have found it, and we'd have less of an obesity problem in the US than we do?
Besides seeing a dietician, cut out junk food and things with empty calories like sweetened juices, pop, etc. Take a daily walk or something to get your heart rate up. Reduce the amount of carbohydrates and sugar you eat and increase fruit, veggies, protein and healthy fats.
I don't like veggies. I just don't. So I find ways that I can like them. My mom always made just plain green beans steamed with some salt and butter. Then I met my H and he made them seared in butter in a skillet with onion, garlic, salt and pepper. So much tastier! I don't really like salad, so I started searching around and trying a zillion different dressings to make salad more palatable for me. I stir fry veggies in sesame oil and soy sauce or liquid aminos. Mix it up and find ways to like healthier foods.
