rh negative and subchronic hemorrhage

I am just wondering if anyone has an success story for having both of these things during their pregnancy.  Last night I had a little bleeding (I am 7 weeks and 1 day)) I called the advice nurse and since I had not had my first appointment yet (its not until 4/30) to go in incase the baby is implanted incorrectly.  During the ultrasound they found out I had a subchronic hemorrhage and during blood work found out I was rh negative so they gave me a shot for that ( I should mention I have miscarried in the past and the er doctor told me i should have been tested or given a shot for rh negative because my body probably made antibodies to fight against the rh positive blood I may not be 100% on everything she said i was so exhausted and still am lol).  My regular doctor of course is out of the office until the day of my appointment but I had a call in to her office to see if I should come in earlier to the person filling in for her shes a nurse practitioner  (the ER doctor suggested I go back in two days)  Anyways the ER doctor kept telling me she is an ER doctor and its not her specialty so its best to go to my regular doctor.. it felt as almost a side note but she was explaining to me that miscarriages are not my fault and if I miscarry there nothing i can do and people in my cases experience more miscarriages than women without, almost like she was prepping me that I was going to miscarry, she also gave me paper about miscarriages   I asked her if I was miscarrying or if she thinks i am and she said the percentages would increase my changes but again she thinks i should see my regular doctor for more details.  It felt to my husband and myself that she just didnt want to be the one with the bad news.  Of course I am panicking I emailed my doctor (well the nurse practitioner thats filling in for her since they are not open yet lol) but  I am hoping to hear your experiences with this.  Sorry if I rambled a little we spent 8 hours in ER last night (crazy busy night apparently) and I had to go to work 2 hours later so I am a little tired.  OH I should also add they did see the sac they said but no baby yet they said it could be too early still since my period is irregular.

  • I am no doctor and i am only sharing my experience and what i have been told. The ER doctors do not specialize on pregnancy related matters that’s why they always give a generic response. They will always refer you to your doctor until your far along to he admitted to L&D. Is there another on call doctor in the office you can see?

    I had an SCH around the same time with this pregnancy and it cleared on its own. The doctor told me it did not increase my chances of MC. I spotted for a few days and was nowhere to be found at my 12 wk appointment. Regarding RH stuff, my understanding is that it’s a concern whenever your blood could mix with baby which is usually at delivery. I wasn’t tested until later in my pregnancy with my son. Google might be able to clarify that science in the meantime.

  • No experience with the SCH but I'm Rh-. With my loss my blood was tested when I started spotting and they learned I was Rh- so I got the Rhogam shot. The fact that I was Rh- had no impact on my loss. It would have happened either way.

    With DS, I had no spotting so I didn't need a shot early on. It's routine procedure (at least at my OB practice) to give a Rhogam shot to Rh- mothers around 26-28 weeks so I got one then. After delivery they'll usually test baby's blood and if baby is Rh+ you may get another shot. If baby is Rh-, there's no need since you're both negative.

    With an SCH and Rh- blood type it makes sense to me they would give you Rhogam now just to be safe.

    An ER doctor is generally not going to give you specific results either way on your pregnancy. That's not what they specialize in so they will always refer you to your regular doctor.

  • I'm RH- and have had two successful pregnancies. This pregnancy I had 2 or 3 SCH, but did not bleed and they resolved on their own. 

    ER docs are notoriously unhelpful with pregnancy related issues. They will always send you back to your OB. I'm sure the nurse was covering her bases telling you to go in especially if you were having pain on one side. Plus if you're RH- and bleeding you do need to go get the Rhogam shot.

    Your bleeding could very easily be from the SCH, especially if it stopped after a few hours. I've seen a number of women have very heavy bleeding with clots from SCHs and everything turned out fine for them. Hopefully this is the case for you. Best of luck!

    So sorry you are going through this. ER care really leaves a lot to be desired when it comes to pregnancy.

    I had multiple SCHs with my 3rd pregnancy. Depending on the location of the SCH, it actually CAN increase your risk of miscarriage, unfortunately, especially if it is large. The first couple of SCHs I developed were not near the placenta and did not pose too much of a risk. I would bleed, and then they would resolve. My last SCH was large and developed between the placenta and the endometrium and that one caused me to miscarry at 9 weeks.

    As for the RhoGAM, my doctor said the current protocol is to give it any time there is bleeding in pregnancy at 7 weeks or later. The ER actually gave it to me when I started bleeding with my first loss (described above) around 5 weeks, and again at 6 weeks with my second loss. My third loss was a chemical pregnancy so I didn't end up at the hospital and just called my fertility specialist to ask if I needed it. She's the one that said RhoGAM is not needed before 7 weeks. Better safe than sorry, I say. Hopefully you are able to get in to see your OB soon to get some more detailed care. GL! 

    ETA: If you are worried (wrt to a previous miscarriage and not getting RhoGAM), you can also ask your doctor for an antibody screen and they will be able to tell you if your body makes antibodies to Rh antigens. I've had one done.
