has anyone had issues getting drs to complete intermittent FMLA forms for appointments? 

Due to being considered high risk I've had a lot of dr appointments already, including a Maternal fetal specialist my RE referred me to

my RE's office filled out FMLA paperwork but said they would only account for appointments with their office and the MFM office said they don't fill out FMLA forms that it needs to go through the referring office .I've seen them twice now and if I can't get these visits certified as pregnancy related and protected under FMLA I'm going to lose my job and my health insurance. 

I don't understand why this is so difficult

You should only need one office to fill out the paperwork to open a FMLA case for high risk pregnancy. Once the case is open all appointments for this pregnancy regardless of which physician is seeing you should be covered under this case. You would just need a note from MFM that you were seen in office on the days that you go. I work in HR and companies can require documentation but not certification from each doctor.

I feel your pain. When I went on leave, I was just a couple weeks shy of being st my job for 1 year, so they couldn't approve FMLA, which means I've bee on unpaid leave. This is so frustrating.
