C-Section Paperwork-Advanced Care Plan

I'm wondering if people are willing to share their experiences about whether or not they have chosen
to write an advanced care plan (it's optional where I live), or other (scary but recommended) just-in-case documents prior to their planned c-section? 
I've tried to broach the subject with my husband and freaked him right out the door...
