Vaccine survey

I am doing a survey for a class I am taking right now and it would be great if anyone could take it ! It is anonymous and just for school use. If a question doesn't apply to you just choose other. Thank you all!

    Did you write the survey? There are a few glitches, just to let you know. I answered "yes I will vaccinate on schedule" and the next page asked "why are you not choosing to vaccinate on schedule?" Also, to the question "which of these diseases have you contracted" there is no answer choice for "none" only "I'm not sure" which is not the same thing. So you may want to do some editing to make sure it isn't spun towards not vaccinating (which is how it read to me...maybe I was overthinking the meaning of the glitches).

    Definitely not trying to be rude...just trying to offer helpful feedback!

    I had the same glitch where I chose yes to vaccinating and it prompted me to answer why I had not

    You don't have to answer those questions. I just left them blank.
