after delivery they'll have someone come with the paperwork and walk you through it. if you know who the father is, I think he can sign, but don't know that it's required.
It varies from state to state. Normally it is done with the birth certificate, some states you need to go to an office in person, but I think some states you can mail in. In the hospital they will have your baby listed as "baby boy/girl Lastname" and they will probably give you paperwork to do the birth certificate.
I cannot remember ANYTHING about the paperwork! I do remember that we didn't do anything until our baby was born, and then my husband mostly dealt with it. But there isn't anything to do ahead of the birth. You get forms for the birth certificate, social security number...some insurance stuff, too. I don't think both are required to sign off on the birth certificate, although they will document the name of any known father.
In canada you must fill out the b.c information before you leave the hospital. No, my husband was not required to sign it, but his information was on it.
All I know is that there was a TON of paperwork afterwards. One of them is where you name the baby for the birth certificate. My daughters has both our information on it but since the father and I weren't married he also had to sign a separate declaration of paternity form which came in handy when I had to take him to court for child support.
