Pregnant after IF

When was your first MFM appt?

Just curious when you were all referred to MFM (if you were). I'm 13w1d with twins, and I've been told that I WILL be referred, but not when.

Me- 35, mild hypothyroidism

DH- 29, low count due to a chromosomal abnormality, only option is IVF/ICSI with PGD.

Married 5/13

TTC since 8/13

IVF/ICSI #1 ER 9/14 - 14R, 6M, 6F, 5 blasts off to PGD- 1 normal female, 2 balanced males

FET 12/8 of 2 frosties - 1 male/1 female - stick babies stick! 

Beta #1 10dp5dt 444! Beta #2 14dp5dt 2,340!  U/S 1/5-- TWINS!!!  EDD 8/26/15

    I was referred for my NT scan at 12w2d. I didn't see them again until our anatomy scan, and since then I've seen them every 4 weeks.

  • I went to my first one at 12w too. They told me to schedule the anatomy one at 18-20w there also.

    Long of the Short:
     TTC since April 2013  DH 42 y/o I'm 30  Dh had vasectomy reversal Feb 2013 after 3 months developed scar tissue 
     First Re appt was September 2013 OOP for everything minus meds  
    DH's TESE surgery December 2013
    First cycle was February 2014 BFN none to freeze
     Second cycle was April 2014 BFP ending in Chemical Pregnancy none to freeze
     Third cycle June 2014 BFN none to freez
     Fourth cycle October 30th 28 retrieved, 13 mature and 12 fertilized
    PICSI, assisted hatching and fresh sperm from my DH's TESE surgery used
    5 FROZEN from a freeze all cycle! 
    FET completed on DECEMBER 9TH!!!

    First Scan January 5th! Everything looks great! Heart rate of 121!
    Second Scan January 20th. Baby Justone13 looks amazing. Heart rate of 175!

  • So typically can't start with an MFM if you aren't high risk yet, right? You start with an OB and then get referred to MFM? Just curious...

    @CML11‌ that's correct. If I wasn't pregnant with twins, I would have had my NT and anatomy scans with a tech in my OBs office. But due to the twins, SCH, and previa, I'm now working with both and will continue to for the rest of the pregnancy.

    ETA- some offices don't do NT/anatomy scans themselves and will refer to a MFM just for those scans.

  • I first saw MFM at 12 weeks. I was referred to evaluate the use of lovenox that the RE put me on. I assumed I was going to be dismissed (and so did my OB) but they want me to come back at 19w for the anatomy scan.
    Me: 37, DH: 38: ttc 7 years, dx: unknown
    10/11: after 2 years, saw a RE, FSH 5.4
    11/11: BFP! (surprise after thyroid & normal hsg),
    12/11: missed m/c after 7 week u/s, 1/12: D&C
    6/12 IUI#1-IUI #3: clomid = BFP!, C/P
    IVF #1(10/12) FSH 5.4, AFC: 16 long Lupron, 5R/5M/4F, all 4 made it to 5dt, 1 blast/1-8 cell transferred=BFN
    IVF #2(12/12)AFC 21, MD lupron, 4R/4M/3F, 5dt of 1 blast and 2-8cell. BFN.
    IVF#3(4/13) Natural start antagon protocol, 12R,11F. one PGS normal at day 6 transfer. BFN.
    IVF#4 (11/13) C.CRM (ODW.U normal 8/13 Still no Diagnosis) EPP/antagonist. ER 13R/7M/6F. Only 1 made it to freeze. Abnormal. Looking into options of DE, Fresh vs frozen.
    10/14 new local RE to look into what's next. CD3 FSH 4.7, AMH 0.9. Met with DE agencies and exploring options for feb/march 2015.
    Surprise natural bfp (4 days before donor is signed). Beta #1 at 9dpo: 51.8, 2nd beta: 195 (25 hours doubling) @11dpo. 3rd beta (12/15): 516 (35 hrs doubling) 4th beta(12/17): 895 (58 hours doubling) 5th beta(12/19): 2120. U/S at 5w0d(12/22): one gestational sac with yolk sac. U/S #2 (6w0d)12/29. One little bean measuring 6w0d with HR 124. 3rd u/s(1/4)7w0d: baby measuring 7w2d. HR 134. 3/30: A/S at MFM went great except for low lying placenta. Verifi results are normal! Team Blue! Please send any positive thoughts our way! EDD:8/24/2015

  • I think I saw my OB at 10 weeks, and at that appointment, the referred me to see my MFM at 12 weeks for my first tri screening (NT scan and some blood tests).  I then had an appointment set for every 4 weeks with my MFM, as well as every 4 weeks with my OB, so I was seeing someone every 2 weeks.  

    I'm surprised that with twins you haven't had your referral yet.  Did they give you the name of an MFM for you to call?

    2 rounds ovidrel with timed intercourse (no result): 3 rounds IUI with clomid + ovidrel (no result)
    2014 summary (to date):
    IVF cycle 1 - ER: 4/17 (28 follicles, 3 fertilized, 2 survived to day 3);
    ET: 4/20 (3rd day, 2 embryos - 1 @ 6 cells & 1 @ 4 cells); Beta 5/1 - BFP!; 
    1st scan 5/13 - development behind, no heartbeat detected; D&C 6/2; WTF 6/13
    IVF cycle 2 - BCP begun 7/12; stims w/ HGH begun 7/26; 
    ER: 8/6, 12 follicles, 7 fertilized w/ ICSI; ET: 8/11, 3 blastocysts left, 2 transferred, 3rd arrested 8/12 - none to freeze :'(
    1st beta 8/19 - BFP! 294; 2nd beta 8/26 - 4976; 1st u/s 9/2; 2nd u/s 9/9 - two little heartbeats at 140 each!!! 
    EDD: 29-April-2015
    Other Meds: Synthroid 100 mcg/daily

  • @AMShark‌ Not yet. I won't do a NT since we did PGD, so I think that's primarily maybe we I haven't yet. I do have an appt tmrw, and will ask now, seeing that so many have been referred by 12 wks.

  • I saw my MFM at 12 weeks for the NT scan, and Genetic testing. I will be seeing them every 4 weeks.My schedule is like AMShark's, seeing someone every 2 weeks.

    My RE didn't release me until 8 weeks for fear of vanishing twin. I think I first saw my MFM as early as 10 weeks. At that scan, they scared the crap out of me when they saw thickness of Baby A's head and told us it could be DS. Had the Maternity 21 test that day. Came back at week 13 to find out that test was negative! Since then, MFM was seeing me every 4 weeks to keep a close eye on babies with hour long US's each time.

    OB visits were also every 4 weeks and pretty much a waste of time. OB blew off all my concerns and might have looked at my stomach, nurse took vitals but did check HB's with Doppler (which was reassuring) and nurse usually answered my questions.

    Now, I see my MFM every morning since I'm on hospital bedrest.

    1/30/2015 PPROM at 21 weeks, 3 days. Hospital bed rest to let these sweet peas cook. Holding on for 34 weeks!


  • I saw my MFM at 12 weeks for my NT scan because my OB office doesn't do that scan. They then had me do my A/S there instead of their office because of all my bleeding issues which made me high risk. I saw MFM for my fetal echo a few weeks after my A/S and will see them at 31 weeks for a growth scan, but I don't have regular appts with them.
  • My OBs office does not do the ultrasounds at all with the twin pregnancy - all ultrasounds will be through the MFM office.  I saw MFM around 12 weeks for the NT scan, but was back at 14 weeks and 15 weeks with bleeding issues.  MFM believes placenta previa is irritating my cervix and causing the spotting.  I have another scheduled appointment with MFM at 19 weeks for anatomy scan, and at 22 weeks for the fetal echocardiogram.

  • I asked my OB at my first appointment if/when I would be referred to MFM. She said I would not be referred unless there were issues. (Not sure how they would identify "issues" as appointments have been quick in, body weight, how are you, check heartbeats, and urine/blood.) Last time I acutally had to ask them if they would take my pee, as they apparently weren't planning to ask for it. Pretty sure they never did anything with it as I never got results. As @starlightgator‌ said, feels like a waste of time. I elected to have the NT scan done, which apparently my OB sends all her patients to the MFM department for their techs there to do. Thankfully, when I went, the MFM pretty much decided I was going to be coming to see them monthly because of the twins. So, I was never actually referred, but am so glad the MFM seems to take twins more seriously than my OB. I would have looked for a new OB but my insurance makes that complicated and I'm technically being seen by a big group of OBs anyway. Whichever one delivers will be chance unless we end up scheduling a c/s. I love the MFM though so I feel like I'm being taken care of. IDK if seeing MFM for every twin pregnancy is overkill, but it makes me comfortable, so if I were you I would push for the same. Good luck!
