HCG Levels for twins at 7 weeks

So my doctor says my HCG levels are very high but when we do an ultrasound we are not seeing anything yet he said by my numbers and by my period we should be seeing something another doctor I saw said he thinks I may be earlier than 7 weeks I have a lot of twins in my family. Just curious on some thoughts my HCG levels were 6,000 last week.

  • One HCG reading doesn't really tell much (it's the doubling pattern that does) and while it is true that twin pregnancies generally have higher levels, it isn't always the case.  With that said... I don't think 6000 is that high.  I'm fairly confident that I'm only having one (ultrasound is on Thursday).  I'm 7 weeks today and last Wed. my level as 23,000something (which is right in the middle for 6 weeks according to this https://americanpregnancy.org/while-pregnant/hcg-levels/).  I believe that you're supposed to be able to see something on a transvag ultrasound starting at around 1000.  Hopefully you're just earlier than you thought and something will show up soon!

    8/14 - Bloodwork showed heterozygous MTHFR gene mutation and elevated AMH (but no cysts)
    9/14 - HSG, Hysteroscopy and laparoscopy showed tubes clear, but found and removed Endometriosis, scar tissue and 2 small benign masses on my tubes.
    11/14 - Daily ultrasounds confirmed that my follicles are not releasing the egg and every other day blood draws showed very low progesterone... DH's SA was near perfect
    12/14 - moving onto meds!!! Hold up!!!  Surprise BFP after only taking the progesterone!  Now what do I do with all of the other meds we already paid for?!  Due at the end of August 2015 Beta#1- 4434 @ 22DPO  Beta#2 - 7335 @ 25DPO Beta #3 - 14429  @ 28DPO

  • They have been taking my blood every other day and they said the way it's going up its more than doubling. Everyone I have talked to has been able to see something at 7 weeks. I really am just hoping I am to early to see.
  • I had a trans vaginal ultrasound and they still haven't seen anything :( the doctor I'm seeing I feel like has not really been doing much about it either I have been very worried.
  • From my last period I should be 7 weeks. Is that always accurate? Could I just be to early? Iv known I was pregnant sense I was 4 weeks.
  • Yes it was done the same day he said something's was weird that he couldn't see anything and that really scared me. So he scheduled me for another ultrasound next week.
  • I'm sorry they haven't seen anything.  It must be hard being in limbo.  I am an IVF patient so I have already had 5 u/s so far with this pregnancy.  I would guess that by 7w they should be able to see at least a ges sac, with a yolk sac and fetal pole.  I would even say there should be a hearbeat but if your dates are off even by a few days then this would change that. 

    This seems to be a wait and see type of thing.  Hang in there...

  • I know how you feel. By my last period I should be 7 wks. On Friday I had a transvaginal u/s and she wasn't able to see anything either, but since I'm EBF my 10 month old DD they said that my dates could be way off because I haven't had but 2 periods (one on which we obviously conceived) :) . Most likely I'm only about 2-3 weeks so we wouldn't be able to see anything. Have another u/s next Monday. Limbo stinks :-(
  • Id feel so much better if the doctor didn't tell me something was weird about it. With my numbers being that high and by my last period. This is my first pregnancy and I'd feel better if I knew it was a little more common or normal. Just hope everything is ok and goes well.
  • He said I would be in a lot of pain so they never even checked I have also thought that myself. But I have had no cramps at all. And haven't been in any pain so he said he doesn't think that it is.

    Best of luck to you and I think I'm safe in saying that we all hope that you get good news at this next ultrasound.

    It sounds like your Dr. has failed to explain things to you in a way that you understand.  I would encourage you to ask some questions.  Your Dr. should not be using words like "Weird."  S/he should be saying "This is what your test results are and this is what I'm seeing.  This could indicate any of the following things, and here is how we would handle each of those scenarios."  S/he should list things out for you and ensure that you understand prior to leaving the office. 

    I'm so sorry that you're in limbo and I truly send my best wishes for a viable and healthy pregnancy.

  • Did he rule out an ectopic pregnancy? I don't want to make you nervous but if I were you I would have wanted to make sure my doctor ruled that out. 


    This is what I was thinking or a blighted ovum.  I know with blighted ovum's numbers continue to rise but nothing develops.

    OP, can you call your dr. and ask about these things and try to get more out of him other than "that's wierd"?

  • I just moved and I had a wonderful OB were I was before but now I am way to far to see her I moved from New Hampshire to Arizona and the doctors I have seen out here are nothing like the ones in New Hampshire I feel like they are not taking care of me the way they should I want more answers and I want to know if my baby is going to be ok I called again today at 9am my time and they said the doctor will call me back around 5pm that doesn't make me feel better I feel like I am wasting time and there is no other doctors close to me. My fiancé and I have even thought about moving back to New Hampshire over this.
  • In the ultrasound they don't see anything at all looks like I am not pregnant that's why they keep taking my blood to make sure my levels are still going up and they are my levels keep rising like they should just they can't see anything
  • @Leslie1129‌ Where are you in AZ? You must be in a small town? We have plenty of great docs in phoenix. Let me know if you need any recommendations.
  • I'm with @MeatandBandP on this one.  If you've known for 3 weeks that you're pregnant, your numbers keep rising, and they can't see anything in your uterus that shows you're pregnant, they need to be looking elsewhere.  I pray that's not the case for you but I feel it's irresponsible of them to not look.  Not all ectopics are accompanied by pain; I have friends that can attest to no pain until their tube exploded.  It's just not worth taking the chance. 

    T&Ps to you that everything is going great in there and that your next ultrasound shows a perfect ute-dwelling baby!

  • I am 3 hours from Phoenix I am in Kingman AZ. I have another Ultrasound on Thursday and my last ultrasound was this past Wednesday. Do you think by Thursday they will be able to see something? If it is in my tubes how do they tell us that by ultrasound as well? I can ask them to check my tubes. I don't know much about this at all :(
  • Best case scenario: You ovulated a lot later than you thought, but even then, if you've had a positive test for 3 weeks, they should be able to see something, even if it's only a sac. They should do a transvaginal ultrasound and check your uterus and your tubes to try to see anything.

    Generally, from what my doctor told me, by the time you hCG levels hit 2000, and definitely by 5000, they should be able to see something, even if it's just a collection of fluid. If they were 6000 last week, if the numbers have risen appropriately, they should definitely see something somewhere. After a certain point, however, hCG numbers become much less reliable for dating or diagnosing pregnancy than ultrasounds. I hope that you get some answers with your next appointment.

  • @Leslie1129‌ oh wow kingman... you are way closer to vegas. Definitely make sure they check your tubes on thursday. If you are still unhappy with your doctor after your next appointment and don't have answers, please seek a second opinion somewhere, even if it means driving to Vegas or going to the ER. A possible ectopic pregnancy is one of the few ER worthy situations in early pregnancy. My hope is that your doctor was so bad that maybe your LO is tucked up in your uterus and they just didn't see it. Weird things have happened before that resulted in normal pregnancy, so try to stay positive and you have my T&P!

    My HCG was over 7,000 at 5 weeks.... we saw nothing as well, Well, we saw a GS. HCG levels can't really indicate twins or a singleton.

  • I hope you have some answers soon. I don't have anythjng else to add except to make sure to advocate for yourself. You deserve better answers than 'hey, this is weird' when dealing with pregnancy and fertility.

  • I just want to thank you all. I have thought about going to the ER just because the suspense is killing me and if something is wrong I want to know and I don't want it to be to late when they find out. I really do hope and pray that everything is ok and somehow they either didn't see or I am to early. This is my first pregnancy and my fiancé and I tried for over a year to get pregnant so when we finally did it almost didn't seem real and we were so happy I truly hope everything is going to be ok.

    A gestational sac is not nothing. 
    I meant, nothing, as in, no fetal pole/baby.

  • My hcg was over 10,000 at 5w1d. And two days later it was over 17,000. I've had an us around 7 weeks confirming gestational age was accurate and that there is just one baby in there.
  • I would press for more information, doesn't seem like he was very with thorough with you and that would worry me for future care. I would look at 2nd opinions. even if it meant drive up to an hour away. It would be worth the peace of mind. 
    An ectopic pregnancy can be very dangerous, not to scare you, but at any hint of pain (not just a cramp) I would be headed to an ER because you wouldn't want to lose the tube.

  • I would keep pressing for more information, and I wouldn't want to wait a week for another ultrasound. As others have said, you should be able to see something in your uterus with a transvaginal u/s once your hcg reaches around 2000.

    Your doctor is wrong that you would definitely be in a lot of pain with an ectopic pregnancy. I had none with mine.
