Request Letter for Eval

Okay so the first time I requested an eval through the school district, it was by word of mouth, they sent me the consent forms in DS's backpack and started it 60 days from the date of consent.

DS is up for the triennial eval at the school district and I plan to write a letter to the diagnostician and school teacher with certified mail.

What do I need to include in my letter? I have writer's block and have till next Friday to send it off.
