I have to take all of my leave under FMLA, as my company doesn't offer any other maternity leave. My boss just told me to write him a letter requesting my leave, has anyone else had to do this? I'm having troubles finding out how to word the letter, and what I need to include in it. I tried google but I am not finding much. Thanks.

I believe that I have to type a letter as well. I haven't started mine, but I think I am just going to alter the wording in my letter that announced my pregnancy. Basically just stating the dates that I will be out and such...and thanking them for their support and flexibility during this time.

The Letter: I would just make this short and sweet, noting your condition, your due date, naming your medical practitioners and of course, sign and date it. Maintain copies of everything for yourself. I'd also keep track of formal and informal correspondence between my supervisor and HR department, just to be safe. (Hey, I trust no one!)
Did a quick google of FMLA and there *is* a form recommended for use, to be completed by your practitioner. My company requires this a month in advance unless an emergency happens (i.e. early delivery, ptl) and then, ASAP. I will be completing this Wednesday when I go back to work.

My work required the US Dept of Labor form to be completed & signed by doctor. They also have an internal form where I request the dates I want to take for FMLA, basic reason. Then my manager signs off and HR signs off.
If you have to write a letter, I'd keep it short and simple. They probably just want something in writing with your leave request.
